Fantastic staff, it is easy to see they really care. No place is perfect but this facility is wonderful. They have a great therapy department!

Edit to Add: because of their carelessness and lack of attention to my grandpas drs directions. We lost my grandpa after he left their facility in an ambulance. If they would have made sure they had the equipment to care for him and would have actually taken care of him, I would still have my Papa. If I could give 0 stars I would. The care in this place is absolutely terrible. My grandpa hasn’t been here a week and has fallen twice and we’re at the hospital again. He’s suppose to wear a Cpap at night and they aren’t making sure he wears it. His CO2 is 110 and a normal level is suppose to be in the 20s

My dad (w/advanced Alzheimer's) was here. He was not given his meds the first week and it didn't go well. From then on, we got nothing but constant pressure to move him out. He lost about 10 pounds in the month and a half he was there. He declined dramatically but care was not adjusted, so I spent most of my visits providing the basic care he was not getting from the facility. (Changing out of urine-soaked clothing, assistance eating, toileting, etc.) We begged facility to "try really hard" to work with us and help him adjust. The Director of Nursing claimed he was the only Alzheimer's patient they've EVER HAD that wandered; she was sure his wandering was going to trigger one of the veteran residents with PTSD to take him hostage and beat him to death while there was nothing anyone could do. As bizarre as this sounds, she said it all with a straight face. At first available excuse, he was sent to the ER for a "meds check" in the night and we didn't know until the hospital called since the facility was refusing to take him back. Terrified about finding a new placement with no notice, we asked for help (they are legally obligated to provide). The administrator openly mocked us by using the exact phrase we'd used in asking for help earlier, sarcastically repeating over and over they'd "try reeeeeeeaaaaaaaalllllllllllly hard" when we asked what would happen if we could not find a placement immediately. He ended up four hours away. He died 3 months later. It was a tremendous hardship on us, and we were not able to see him much in his last days. But at least he was actually getting the help he needed there. I have no doubt his passing was greatly hastened by the "care" provided Osage Beach Rehab. I hate reliving any of this horrific experience, but if sharing some if it can save even one other family from what we went through, it's worth it.

My mother was in there for almost a month and the service was great. The people worked with her to get her where she could go home. Almost all of employees are great and work extremely hard to make sure that each patient receives the best care.

The only reason they get 1 star is because their facility didn't smell horrible. That's the ONLY good thing !
