Sierra Winds – Medicare Certified Since 1995

Sierra Winds is a Medicare and Medicaid certified skilled nursing facility located in Peoria, Arizona. This facility has been serving the community for 31 years. A Medicare-certified skilled nursing facility provides 24-hour nursing care, rehabilitation services, and assistance with activities of daily living for patients who need skilled nursing or rehabilitation services on a daily basis.

This facility is part of a Continuing Care Retirement Community (CCRC), which offers a continuum of care on a single campus—from independent living to assisted living to skilled nursing care. CCRC residents often have priority access to the skilled nursing facility and can transition between levels of care as their needs change without having to relocate to a new community.

This facility has 70 certified beds. Skilled nursing facilities provide 24-hour nursing care for patients who need rehabilitation services after a hospital stay or ongoing care for chronic conditions. Services include skilled nursing, physical therapy, occupational therapy, speech therapy, and assistance with daily activities.

Staffing Levels: Based on CMS payroll data, this facility provides approximately 5.23 hours of total nursing care per resident per day (2.89 hours from CNAs, 1.08 hours from LPNs, 1.26 hours from RNs). Physical therapy staffing is 0.13 hours per resident per day. This represents above-average staffing compared to national benchmarks.

Medicare Quality Ratings: According to CMS, Sierra Winds has an overall quality rating of 1 out of 5 stars, which is much below average compared to other nursing homes nationwide. Individual category ratings are: health inspection: 1 stars, staffing: 1 stars, quality measures: 4 stars.

Medicare Coverage: Medicare covers skilled nursing facility care for up to 100 days following a qualifying hospital stay of at least 3 days. Days 1-20 are fully covered by Medicare, days 21-100 require a daily coinsurance payment. Many residents also use Medicaid, private insurance, or pay privately for long-term stays.

Questions to Ask: When visiting a nursing home, ask about: staff-to-resident ratios and RN coverage around the clock, how they handle medical emergencies, activities and therapy programs, how they communicate with families, their approach to falls prevention, and policies for managing resident complaints.

Sierra Winds is consistently smooth for deliveries. Security greets us at the entrance, reception confirms building/room details, and visitor parking is available close to the lobby, which keeps the walk short even with larger arrangements. Wayfinding between wings is clear; first-time visits benefit from asking which elevator is quickest for the resident’s wing. Midday tends to be the easiest window—lobby foot traffic picks up around dinner. Handoffs are courteous, and when residents are out, reception logs the delivery so it’s kept safe until pickup. Overall, organized staff and clear access make on-time deliveries routine here.

The facility is impeccably clean and beautifully maintained, creating a welcoming and peaceful environment. Their focus on therapy is truly outstanding — the therapy team is professional, supportive, and committed to helping residents regain their strength and independence. It's clear that the staff genuinely cares about the well-being of their residents. If you're looking for high-quality care in a gorgeous setting, I highly recommend Sierra Winds! Stop in and tour! 5 stars is simply not enough. Sierra winds is like nothing I have ever seen before. We recently moved my mother in law there. She had previously lived in her own home with the help of an occasional care giver. She was not excited about the move until she got there and now she refers to Sierra winds as "The Resort" she lives in. She has all the independence she wants with help whenever she needs it. The staff is incredible, they have welcomed her with open arms and have gone above and beyond to make sure she is living her best life. The apartments are beautiful the grounds are lovely the restaurant and bistro are delicious with so many choices. They have tons of fun activities. We have met so many residents that have lived there for 10+ years, which I believe truly speaks volumes. The residents are constantly stopping by to visit and welcome her to the community. Honestly, it is a perfect independent living resort with the option to add any assisted living care you need.
