LIFE CARE CENTER OF PLAINWELL

I would never recommend Life Care Center of Plainwell to anyone looking for quality or compassionate care. Our experience was absolutely horrific. The facility is severely understaffed, and basic care standards were completely ignored. I would give them zero stars if I could. We would visit as late as 11:30 a.m. and find my dad still in bed, lights off, still lowered to the floor, undressed, and barely covered. It took them a week of repeatedly being asked, for them to help him use his cpap machine.. and then it was sporadic. He was rarely, if ever, helped to move or sit up unless we were there to insist on it. They did not rotate him. The staff failed to shower him as promised and often left him unchanged for long periods. The cleanliness of his room was unacceptable — at one point we even found feces smeared on the trash can. My dad cannot advocate for himself, so he relies entirely on others to care for him. When we finally transferred him to a new facility, we discovered SEVERE bed sores that should never have developed with proper care. We couldn't get him out of there soon enough. This experience has left us completely heartbroken and outraged. No family should ever have to see their loved one treated this way. I truly hope management and regulators take a serious look at the conditions in this facility. Side note: Not all staff were horrible and some had good intentions. They just simply had crap to work with and no support. I would never take my loved one here.

Best place ever. People are so nice and helpful and the most caring and loving ever. Thoughtful and attentive and explain everything in plain easy to understand words. Prompt attention and therapy is fantastic. Rides to doctor appointments are prompt and careful driving. I would recommend this facility to anybody needing rehab and from everything I see it’s a wonderful place for the long term.

My mother is a resident at Lifecare, going into her second year. Overall, the care has been good. Currently there is an outbreak of COVID which has caused a staff shortage. They are following State Guidelines for care. The big concern I have is mom being in bed for the past two weeks. When I've called to ask how she's doing, and ask if she's been out of bed, her nurse for the day tells me she's up and doing good. The truth is, she's in bed. The lack of trust now, has been frustrating. The situation leads me to believe she is not getting showered, is not having her bathroom needs taken care of, has to try to eat while laying in bed. For the most part, in the past, the care has been good.

Hands down, this is the ABSOLUTE best physical therapy facility. I have been trough multiple surgeries...knee, leg, and didn't get the rehabilitation from others PT facilities like I do here at Armour in Plainwell MI. I started coming here after a shoulder injury and the staff is absolutely on point. 💯

We went on a Sunday to visit my Aunt. Looked like very little staff was there. The ones who were working were running their butts off. Many call lights were on including my aunts. I was able to help her. These places always seem to run short of help.
