If I could give no stars I would bt it wont let me. Never put a loved one in this facility. The CNA care is inconsistent. They all do things differently. There are no guidelines. They are short staffed. The management denies it but the care givers admit it. The food is served cold because it sits in the hallway. The care plan was never really implemented, the laundry gets lost even with a name printed on the clothes. My mom fell many times and their response was that my mom has a right to fall. She would ring the bell and they would take so long to come she would have an accident. My mom weighed 100 pounds and she was in a pull up with a 44 inch waste. She became basically bedridden with dementia while in this facilty. They dont clean the rooms or wash the sheets. My My mom had a stain on her sheets at the foot of her bed where my granddaughter spilled her coke and it was still there 1 month later! We actually had to make notes for the staff and tape them to the wall so each shift would know what to do and expect. I could go on and on but I think you will get the jist of this. For 10 grand a month it's a joke !! The great news is that we moved nom to The Springs. She lives in her own apartment , has gained 20 pounds and is cognitive again! I know in my heart she would be dead now if we had left her there. I give 5 stars to the physical therapist and Mike you were awesome. Thomas......Jo and the manager with the pierced tounge (I dont remember his name) should be replaced!

I wish there was a way of giving no stars at all! Never allow anyone you love to reside in this facility. They constantly make excuses as to why their patients are waiting for their food, (the food sits outside their rooms getting cold while the employees are laughing and visiting among each other), only get bathed twice a week maximum, linens are not changed on a regular basis which is terrible for anyone but especially people who are already dealing with health issues, don't have time to go for walks down the hallways (even though exercise is a portion of some patients rehabilitation and circulation requirements), aren’t able to prepare diabetic friendly foods for diabetics (the cook gave a poor attitude and said they did not have time to prepare seperate meals for everyone)...all because they are under staffed! If you cannot take care of the patients and their specific needs appropriately and within a timely manor then you should not be accepting any more patients and should hire additional staff. This facility neglected my Mother in many ways, including the above mentioned. She was feeling sick and had a terrible cough. When my Mother told them they listened to her lungs and said it’s just a small cough/cold. She ended up passing away and it turned out that she actually had bilateral pneumonia! She was getting ready to come home, so this came as a shock to our entire family! They terminated the only staff member that genuinely cared about the patients due to her needing to be off for her children getting sick. I understand that Polson is a small town with minimal options but I would recommend somewhere else if at all possible! I will be praying for all of the patients at that facility and for their families that they never have to experience this pain!

If you love your love one, DO NOT place them in this place. Visited there a month ago, and it was scary. Nurse Mark had alcohol on breath, and possibly smoked something. They are short on help here! I asked different friends that live in Polson about Polson Health & Rehab, they recommend St. Joseph hospital assisted living, or a place in Ronan. I gave 1 star for the two kids, a light brown hair young girl and tall black hair young man working there who my friend says are very helpful. I didn't put my real name on, because I'm afraid for my friend's treatment.

Excellent staff. And the best therapists hands down. Was there in may to july this year after a stroke. Best experience and walked out. Great food excellent nursing care. Lovely residents. If you truly want to heal this is the only place to go. only b b mm.

The staff is friendly and caring, the patients seem happy and well cared for...the food is quite excellent... and the kitchen staff is proud of what they do. If you have to go to a nursing home or rehab center.. Polson Health and Rehab is a good place to be..
