My husband had a stroke five months ago he was in the hospital and then sent to Perry Creek Nursing Home. I wanted him to go there because it’s close to my house. I see him almost every day and for the longest time we were having a lot of problems there they wouldn’t change his diaper. They wouldn’t help him if he was hungry or thirsty. I called the Imbuddsman. We had a meeting and they did a lot of changes within the therapy place and the business place and I do see a huge amount of improvement lately. My husband is there and he’s basically pretty happy. . He doesn’t complain and he gets a lot of what he wants a lot of the time… So I am glad that he is there now not all of the people are great some don’t care, but there are some good ones there we are working on his Physical Therapy he doesn’t feel a lot In his left side. I’m hoping with Therapy he will be able to get out of the bed and into a wheelchair so he can come home and eventually maybe walk… If you pray, please pray for him… Thank you very much

I visit my husband, Robert Williams at different times on different days. After several weeks, I have never seen any of the staff without a big smile and greeting! Bob is very happy there. The floors are spotless, trash always emptied, patients always dressed neatly and are actively getting about. Kudos to everyone from reception to maintenance! Y’all are doing a marvelous professional job!

I have seen improvements in the facility and patient care. Facility sometimes has to be reminded to clean the residents floor. Really like residents are eating in the dining room and socializing in the recreation room.

Went to visit my relative and speak to social worker. Social worker Taylor has been very helpful. Also while visiting my relative, a CNA named Priscilla was very helpful in assisting him with his laundry.

I would like to take a moment to express my gratitude to the new administrative and nursing team at Perry Creek Rehab rehabilitation center. Thank you for their compassion and care towards my loved one. The nursing staff has been a tremendous support and I am very satisfied with the quality of care provided additionally I appreciate the prompt response to a serious incident that occurred during my visit thank you for your attention to this matter and how effectively the facility manage the situation.
