Posting on behalf of my wife: My dad has spent almost 3 months at Sunnyvale Post-Acute after his hospitalization.  He received a ton of care there: Physical therapy, Occupational Therapy, Speech Therapy (including Swallowing), and even music & aromatherapy.  He has improved to the point that we can bring him home soon.  The primary doctor saw my dad once a week, and the doctor of rehab saw my dad twice a week.  Then there were so many nurses, from RNs to LVNs to CNAs (it is a skilled nursing facility after all)!  They also individualize the meal plan per the hospital discharge orders.  For my dad it was low-carb vegetarian, and pureed food only, with 1/1 feeding assistance.  They also crushed medicines.  The staff was amazing: not only are they highly skilled, but they have a lot of empathy towards the patient.  They even got to know the family, as my mom & I were there a lot.  We also enjoyed meeting the nursing students.  A group of the them came to my dad's room to sing Happy Birthday on his 90th birthday! This was our first experience with a SNF, and should we need one again, we would request this one!

My husband has been here several weeks now and has been making progress toward regaining enough mobility to come home hopefully soon. The staff have been top notch in caring for him, arranging transportation to medical appointments and in every other way. And everyone has been super responsive to any concerns we expressed.

Night time crew are mostly lazy sitting chairs watching their smart phones. I asked blanket to the guy sitting chair answering "I'm not your nurse " Asked another nurse, it was 5 steps from the guy's chair. Patients staying here are difficult to speak so nurses don't have the responsibility to patients. I was transferred from Hospital , nurses qualify are big difference . Some Assistant nurses are not nurse like security or janitor. But some of nurses are very good.

I am a patient here and everything went really well. I am getting discharge and it is hard to say bye to everyone. From the nurses to the therapy staff, everyone has been so helpful. The therapy team helped me get back on my feet and were very patient with me. The facility is very clean and odor free. I will miss the staff here.

My husband was at this facility for almost 8 weeks recovering from a heart attack and major heart surgery. This was a difficult time for our family and I was thankful to meet Jessica when I visited the facility for a tour. The place smelled clean and the staff were friendly. I had visited a post-acute facility prior and it was HORRIBLE. Jessica made me feel comfortable and at ease to have my husband continue his care at SPAC. During his stay the CNA’s & Nurses were helpful. It was not always smooth; however, once we voiced our concerns and needs, they responded quickly. Angelica is a good Case Manager. Thanks so much Angelica for all your help and guidance. The Physical Therapy Team (GG, KK, Angie, Brandon, Camille, Jason, Joe and all the others) are AMAZING, a 10++++++ and everyone is very nice and helpful. They know what they are doing!! We cannot thank them enough for all their help and teaching us everything they did over the 8 weeks. We recommend this facility for post-acute care. Thanks again to everyone that assisted in the recovery process.
