Everything is fine until you try to advocate for your loved one. Instead of having adult conversations they become very defensive and find reasons to avoid discussing any perceived conflicts. My poor friend is an 80 year old female and after insurance cut her off on 10/31, they deemed her appropriate for discharge. She was in fluid overload and was gasping for air. I challenged the discharge and filed an appeal with Acentra on 10/31and they completely ignored it. Nov 1 they attempted to discharge and had her sign papers (which they soon shoved a AMA form in front of her and without explanation said "here sign this"). I had to call 911 for her respiratory distress and she was taken by ambulance to St. Joes WHERE SHE WAS ADMITTED for respiratory failure and was found to have Copd exacerbation, ascites, CHF and edema from her toes to her chest. As I write this review she remains at St. Joes as they are aggressively treating her fluid overload Avamere helped create and did nothing to help (No fluid restriction, no monitoring intake/output). The LPN's lack critical thinking skills. The ER RN's could immediately see her respiratory distress.Avoid this place at all cost.

I've had two grandmother's stay out here....Both had horrible experiences and terrible support. They left my grandma soaked in urine, both chair and bed. AND this is just one example.....They weren't extremely helpful, and that is not cool at all when dealing with the elderly. If you have another option, choose it fast.

Kudos to all the staff and caregivers at Avamere. They were very welcoming and friendly from my first day to my discharge day. I had a mix of things going on, including heart, blood pressure, and my introduction to the use of oxygen. Each caregiver was knowledgeable, and always willing to help no matter the time of day or night. I would consider each staff member my friend. Best to all of you…with a grateful heart! In a world of local mediocre health facilities, Avamere stands out. The facility is modern, clean, & not hectic. We needed a transitional facility for our son after a week-long stay at Good Sam. He needed 3X/day antibiotics by infusion & these folks handled it perfectly. The care team, dieticians, nurses, and even housekeeping staff are quietly competent - plus just plain nice. This is an easy recommendation for those in need in Pierce County WA.

I had a wonderful experience with Puget Sound Transitional Care. The team was compassionate, knowledgeable, and dedicated to making my transition from hospital to home as smooth as possible. They provided personalized support, clear communication, and were always available to answer any questions I had. Their thoroughness and attention to my specific needs helped me feel confident and comfortable throughout the process. I am truly grateful for their support and highly recommend them to anyone in need of transitional care services
