THREE RIVERS HEALTH AND REHAB

Three Rivers Health & Rehabilitation Center is an “Excellent “ facility!!! Recently, my cousin was admitted there and believe me, total professionalism beyond my wildest imagination was exhibited ! The entire staff went out of their way to make my cousin feel welcomed and comfortable..They provided exceptional care during her stay and maintained an environment fit for a king … clean beyond measure! I highly recommend this facility to anyone looking for a good rehabilitation program! Thanks for your dedicated service and professionalism . It was highly noted and appreciated.

Following having my second hip revision on 9/9/25 I was transported to Three Rivers Health and Rehabilitation Center on 9/12/25 to begin physical therapy and occupational therapy. Ever since the moment I arrived I was made to feel welcome by the administrator, social workers, my assigned nurses and CNA’s. The physical and occupational therapy I received the past two weeks was amazing, and done so by a group of professionals with great personalities that made therapy bearable. I don’t want to list names in case I forget someone, but I feel that all of my nurses, CNA’s, housekeeping and food service should receive the Liberty award. I highly recommend Three Rivers for anyone needing rehabilitation.

My mother was in Three Rivers for almost two weeks for rehab after a hospital stay. The staff was super friendly and helpful. The Rehab team got my mom back on her feet and walking again. Everyone was welcoming in answering my questions and very helpful with getting us set up for home health and outpatient rehab. The staff has called twice to check up on her and offered that if we need anymore assistances that we can contact them for help. Thank you Three Rivers for making it possible for my mother to go home.

Our family was told during a hospital stay my Uncle was ready for discharge, but needed rehab and could not go home! Three Rivers Health and Rebab was at the top of our list. However, more importantly as a retired LTC Administrator I/we knew that his wife needed 24 hour care as well. It was obvious to me at 89 showing signs of Dementia she needed to be admitted with him. Three Rivers became the salvation our family needed in the pursuit to extend, protect and gratify their lives as we prayed for them. The Kindness, Humility and Genuine Love shone by all the staff for our Loved Ones will remain forever in Our Hearts.

My father was recently here for ST Therapy and raved about his staff, so sharing on his behalf: My Nurse Katina- I really appreciate her sense of humor. She was receptive to my needs and made it easy for me to ask for help. She always brings a smile and laugh. She knows her patients well and picked up on medical changes to get me the care I needed in a timely manner. My Nurse Sharon- She is very friendly and communicative. Any time I asked for even the smallest thing, she got it back to me quickly. She is approachable and made it easy for me to ask for help. My CNA's - Audrey and Kayla- They were both very compassionate and caring. They were considerate of my needs, and communicated well with my nurses on any changes in condition to get me the care I needed timely. They were patient with me while supporting me in being more independent. My Therapy Team- Casey, Kelly, Hailey, Brooke. I was up and walking quicker than I could have expected and made great progress. And I got more out of my shoulder than I thought I would. They were attentive and communicated any potential changes in my condition to make sure I got the care I needed. - Ross Metcalf
