Memory Care of the Triad is a licensed adult care home located in Kernersville, North Carolina, specializing in memory care services for individuals with Alzheimer's disease and other forms of dementia. Who We Serve: Adult Care Homes serve older adults and adults with disabilities who cannot live independently but do not need the level of care provided in a skilled nursing facility.
Memory Care Services: This facility offers specialized memory care services, including staff trained in dementia care techniques, activity programming designed for cognitive engagement, and appropriate safety measures. When evaluating memory care options, families should consider staff training, staff-to-resident ratios, security features, and the approach to managing behavioral symptoms.
Licensing: Typically licensed by the North Carolina Division of Health Service Regulation, Adult Care Licensure Section.
Payment Options: In North Carolina, Medicaid may help pay for assisted living through the Community Alternatives Program for Disabled Adults (CAP/DA). Eligibility is based on both financial and functional criteria. Contact North Carolina Division of Health Benefits or visit https://medicaid.ncdhhs.gov/ to learn about eligibility requirements and how to apply. Wait lists are common for waiver programs, so applying early is recommended.
Families researching assisted living options should verify the facility's current license status, review any inspection reports, and schedule a visit to assess whether the environment and services meet their loved one's needs. For more information about assisted living regulations in North Carolina, visit North Carolina Department of Health and Human Services, Division of Health Service Regulation.
Licensing requirements and regulations may change. We recommend verifying current requirements directly with your state's licensing agency.

I wouldn't put my dog in that place , the nurses were yelling at patients, the staff is so unprofessional, and how they speak to those poor elderly people was disturbing DO NOT SEND YOUR LOVED ONE THERE ! THEY WILL NOT BE TREATED RIGHT ! they should shut that place down , I was appalled just by going with my friend to drop off items for her family member!
I highly recommend this facility. Our mom has been in the facility for 2 months and she is happy. The staff is very friendly and professional. Every time we visit the place it smells fresh and is very clean. It was so pleasant to see all the smiling faces of the seniors. They were engaged in all kinds of activities. There is a monthly calendar of events for them such as games, movies, puzzles, crafts, gardening, etc. They are on a great schedule for (B, L, D, and snacks), everyone eats together like one big family. Its so cute. I can tell this is something they all look forward to. Shout out to the Director, Asst Director, Dietician's, Cooks, Housekeeping, Care takers, Nurses and all other Staff. Hats off to you ALL! Thank you for taking care of my mom. Hope to see you again soon. Best Regards, Georgia
At first I thought it was going to be a good place for my mom. When I took her the first day Candice was suppose to show us around and go over things with us. No show she said she would call me the following week never did. She was there for the month of oct. I didn’t like what I was seeing and hearing the people that worked there yell at the patients across the room To sit down and watch tv they wouldn’t do anything with them just made them sit in front of tv all the time. My mom went on 2 occasions that I know of didn’t get a bath but 1 time that week. They wouldn’t help her put on her compression hose everyday. She needed help in the bathroom 3 different occasions and she would call me on my cell phone saying she needed help I would have to call and tell them she was in bathroom needing help. They wouldn’t even give her a Tylenol. I have witness the help pull the patients back down to watch tv. This place needs to be reported in the way they do the patients there. I tried to get Candice to fax over her paper work in oct so I could have her taken out of there and put in a different facility before Nov. and she didn’t do it till Friday week ago. The only time Candice would talk to me it was about the money. I couldn’t talk to her about the complaints I had. If you care about your love ones this is not the place for them. I’m so glad she is not there anymore. She is happy where she is at now. I wouldn’t give it a 1 star but it wouldn’t post without it. They only had a couple of people that worked there was very sweet. The rest of them acted like they had an attitude. I could ask question they acted like you wasn’t suppose to talk to them.
Very dirty, disrespectful to residence, they go unchanged for hours, very Rough with residence and also will force feed food and medication. For example they did not get changed or checked on from 7Pm to 4:30Am because they did a floor strip and did not plan correctly. And not to mention there are no cameras anywhere on the residence at all. As well as the residence escape Dailey and have the same clothes on for weeks. The resistance bathroom is always broken. Needles in the parking lot. PLEASE DO NOT SEND LOVED ONE HERE. And loved one who are still here please check on. They have already been reported. Thank you so much for your time. Watch out for the ED and assistance ED!
The RCC, Noel is very knowledgeable and shows compassion for the residents at the facility. She's considerate and attentive to the residents and care.
