Family Tree Care Homes
Family Tree Care Homes is a licensed housing with services located in New Brighton, Minnesota. Minnesota Housing with Services establishments are registered establishments that provide or arrange for supportive or healthcare services. This is separate from but often combined with Assisted Living facility licensure.
Services may include assistance with daily living activities, medication management, health monitoring, meals, housekeeping, personal care, and coordination of healthcare services. The specific services are determined by individual service agreements.
Who We Serve: Housing with Services serves adults who need varying levels of assistance and wish to receive services in their own apartment or living unit rather than in a traditional care facility.
Licensing: Registered with the Minnesota Department of Health under Minnesota Statutes, section 144D.
Payment Options: In Minnesota, Medicaid may help pay for assisted living through the Elderly Waiver (EW). Eligibility is based on both financial and functional criteria. Contact Minnesota Department of Human Services or visit https://mn.gov/dhs/people-we-serve/seniors/services/ to learn about eligibility requirements and how to apply. Wait lists are common for waiver programs, so applying early is recommended.
Families researching assisted living options should verify the facility's current license status, review any inspection reports, and schedule a visit to assess whether the environment and services meet their loved one's needs. For more information about assisted living regulations in Minnesota, visit Minnesota Department of Health.
Licensing requirements and regulations may change. We recommend verifying current requirements directly with your state's licensing agency.

I am beyond grateful for the loving, individualized, respectful care my mother received at Family Tree Care Homes. The staff live their passion and calling to care for those losing the ability to live independently. Mom was a wonderful baker and loved to entertain. The staff was attentive to her liking her food served hot and agreed with her assessment that dessert was an important part of a meal. The homemade pies looked delicious and ice cream was always available. They also asked for and made recipes favored by Mom. Regular entrees included fish fry, grilled steak, homemade soup and extra special holiday meals. The care provided to the residents was very individualized. Mom’s desire for independence was respected and as she needed more help, this assistance was provided quietly and gently. She repeatedly told us how nice everyone was to her. She found willing cribbage partners in the staff and their little girls loved to visit and brought big smiles from Mom. Family Tree also lives up to its name by welcoming and supporting all the residents’ families. Decisions about care were discussed with family and our desires were respected. I am convinced we couldn’t have found a better place for our Mother to live her last 14 months.
My Grandmother spent the last months of her life at Family Tree Care Homes. The staff is so warm and professional. They kept our family updated with changes and opportunities and encouraged our participation in her care decisions including discussions with her provider. They provided wonderful, individualized meals for my grandmother, and were especially patient when she was increasingly more particular about her taste. They always treated her with dignity and respect. The home is clean, modern and beautifully designed and is aesthetically pleasing with large windows and a beautiful deck where we enjoyed to visit. They take preventing the spread of Covid-19 and the safety of their residents and staff very seriously, which was much appreciated. I would hands down recommend this facility to anyone I care about, and they’d be lucky to have the opportunity to reside at Family Tree Care a homes.
My mother was very fortunate to spend the last few months of her life at Family Tree Care Homes. We know the care she received was top notch at every level. The facility is very nice and well maintained but it was the staff that really made this great for Mom. I said many times it was like Mom was living with family - only getting better care. Lee, Breanna, Madison, and Jake were all very caring and considerate of Mom. It was comforting to see the steps taken to keep things safe during Covid-19 and yet still allow good visitation opportunities. I highly recommend this home for anyone needing care for a loved one.
We moved our mom to Family Tree Care Homes about 10 months ago. Before we found Family Tree, we had experience with a large senior/assisted living/memory care facility and trying to provide for her care ourselves in our home. I can honestly say now that moving her to Family Tree was the best decision we ever made. This is the happiest we’ve seen our mom in years 😍. And I want to emphasize that Family Tree is aptly named — Lee and Breanna really make everyone feel like they truly are family. And their entire staff is absolutely amazing. You can tell that they really care for our mom emotionally as well as physically. From the great food they prepare, to the wonderful job they do with medications and other medical and physical needs, to their attentiveness and emotional support, Family Tree is truly a unique space in the long-term care world. 👍👍👍 Lee, Breanna, and their staff understand that each individual and their family situation is different. If you want to visit for several hours a day, they welcome that. Families that can’t visit as often can rest assured that their loved one is receiving top-of-the-line care in a beautiful home setting at a very fair price. If you have special requests, they always do their utmost to accommodate them. I don’t say this lightly, but when and if I were to require the type of care my mom needs, I would be very happy living at Family Tree myself 🙂.
In the middle of a pandemic, I realized my mom’s living situation wasn’t sustainable and started to look for a new place for her to live. I didn’t just find a place; I found a home! I discovered Family Tree on a Google search, contacted and met with Lee, had a virtual tour of the home, moved my mom, and never once questioned that decision. Throughout Mom’s time at Family Tree, Lee, Breanna and the staff acted consistently with the verbal information Lee had provided at our initial meeting. Here is one example of how they were attentive to the individual needs of the residents. My mom was a practical and frugal woman, but for the past 50 years, she went to the beauty shop every week to get her hair done. Covid obviously put a stop to that. The staff at Family Tree understood how important this was to Mom and figured out how to set and comb out her hair and even did perms as needed. Mom loved when they did her hair; they brought her a mirror so she could admire how she looked. They even did manicures and pedicures which was something Mom had never experienced. In addition to caring for, entertaining, and feeding the residents, the staff was always busy doing the laundry and keeping the home clean. They are masters at completing all the tasks required to run the home in between their direct care for the residents. Although this type of care situation is expensive, I thought the rate charged by Family Tree was very reasonable and never received any charges other than the base rate. I am grateful for the loving and expert care the staff at Family Tree provided and will highly recommend them to any of my friends who have loved ones that need this type of care.
