Lakeside Memory Care

Lakeside Memory Care is a licensed housing with services located in Plymouth, Minnesota, specializing in memory care services for individuals with Alzheimer's disease and other forms of dementia. Minnesota Housing with Services establishments are registered establishments that provide or arrange for supportive or healthcare services. This is separate from but often combined with Assisted Living facility licensure.

Services may include assistance with daily living activities, medication management, health monitoring, meals, housekeeping, personal care, and coordination of healthcare services. The specific services are determined by individual service agreements.

Who We Serve: Housing with Services serves adults who need varying levels of assistance and wish to receive services in their own apartment or living unit rather than in a traditional care facility.

Memory Care Services: This facility offers specialized memory care services, including staff trained in dementia care techniques, activity programming designed for cognitive engagement, and appropriate safety measures. When evaluating memory care options, families should consider staff training, staff-to-resident ratios, security features, and the approach to managing behavioral symptoms.

Licensing: Registered with the Minnesota Department of Health under Minnesota Statutes, section 144D.

Payment Options: In Minnesota, Medicaid may help pay for assisted living through the Elderly Waiver (EW). Eligibility is based on both financial and functional criteria. Contact Minnesota Department of Human Services or visit https://mn.gov/dhs/people-we-serve/seniors/services/ to learn about eligibility requirements and how to apply. Wait lists are common for waiver programs, so applying early is recommended.

Families researching assisted living options should verify the facility's current license status, review any inspection reports, and schedule a visit to assess whether the environment and services meet their loved one's needs. For more information about assisted living regulations in Minnesota, visit Minnesota Department of Health.

Licensing requirements and regulations may change. We recommend verifying current requirements directly with your state's licensing agency.

I don’t have enough words to express my gratitude and love for each and every staff member at this facility. My dad recently passed after being a resident here for three years, and during those three years he has been taken care of in an exceptionally gracious way. Over the years visiting him I have gotten to know the staff very well, and know they take their jobs very seriously. Not only were they loving and caring to my father, they are to each and every resident there. I always felt so welcomed every time I visited, and loved that they almost always had some activity going on. When it came to my dad’s final days, Brookdale went infinitely above and beyond what I would have ever expected; making sure my dad was comfortable and loved. They created the best environment for my dad and for my sisters to all be together and constantly made sure we were ok. These caretakers will always be very special to me, and I feel so blessed to have had them around during one of the worst times of my life. I could go on and on forever. I truly cannot ever thank this facility enough.

Becky was exceptionally helpful assisting me in navigating a move to MN for my parents. It's so obvious that she genuinely cares about our precious elders. It didn't work out for my parents to move in here due to some unforseen challenges from their home state, but I wouldn't hesitate to have them at Brookdale if it were possible. There's so much to like about this facility and the high standards for the staff. You can feel the warmth, the pride, and the love when you walk in. The residents are celebrated. Whatever past problems were, I think Becky has done a phenomenal job making it better, and I am sorry my parents don't get to go here.

(Note: my previous review was removed for unknown reasons so I am submitting again). My mother was here for less than a year before I moved her to a different facility. Brookdale promised me that I would be the daughter again (and that they would take care of her). This never happened. Cleaning of her room rarely occurred! My mother's room had ants on numerous occasions. Her toilet overflowed on an occasion (without them telling me until I arrived). When I entered her room, there was waste material on her bathroom floor that had also spread to the carpet area. UNACCEPTABLE! The head nurse has no business being in charge of care. The first time that I was there with my mom to make sure she took a bath, the head nurse got my mom into the bathtub but couldn't get the door to close. She called for maintenance while my mom sat NAKED in the tub. She didn't even think to provide my mom with a towel or robe to cover her up. (of course I did!!). I would definitely stay away from this facility! I am giving it 2 stars because the Activities Director is fantastic!!

At our wit's end. My mother has advanced dementia but is able to walk and minimally interact/engage with people. We moved her in just over one month ago and have been incredibly disappointed in the care she has received there (if you can call it "care" at all, it is difficult to tell). From little things (not having a name plate on her door despite multiple discussions with those in charge and reassurances about it only taking a few weeks to come in, and never putting her hearing aids in or charging them overnight - again, after numerous discussions and reassurances about it) to more concerning hygiene issues (not showering/bathing her on days she was supposed to, not brushing her teeth, putting her in diapers despite our preference that she not, as she is not incontinent but only needs prompting for toileting) to much more serious issues (losing 8 pounds - from 130 pounds starting weight - during her first month in the facility, and falling at least 10 times - with 2 serious falls involving head injuries). We are just so disappointed in the staff response to our concerns - "oh yes, we'll fix that, it's a transition for everyone!" After one month, you'd think some things would be going better, or a lack of improvement from staff would be appropriately handled. My heart breaks from the carelessness shown for my mom's well-being and health. Please ask for references before moving a loved one into a facility. Please ask how miscommunication and unmet expectations are handled, and after multiple attempts who to approach next for assistance. Please ask about staff:resident ratios. I suppose the silver lining is that we now understand more about what to really look for and ask from a facility during the "interview" process. Smh

We moved our Mother (85 then) to MN from PA last Holiday Season due to challenges with her memory and safety. The original facility in Plymouth, MN was quite the challenge, but also assisted living. It wasn't a good fit. Mom needed more direct care, love and guidance. Our family has found Brookdale Plymouth to be a vibrant community for our mother (singing, dancing, gardening) with individuals who have significant industry experience (very important) and a deep and caring heart. Mom is very happy and we are as well.
