Carlton Plaza of Sacramento

Carlton Plaza of Sacramento is a licensed residential care elderly located in Sacramento, California. Residential Care Facilities for the Elderly (RCFE) provide non-medical care and supervision in a homelike setting for people 60 years and older who are unable to live independently.

Carlton Plaza of Sacramento has a licensed capacity of 185 residents. Services include assistance with activities of daily living (ADLs) such as bathing, dressing, grooming, and medication management. RCFEs also provide meals, housekeeping, laundry, social activities, and 24-hour supervision. Many offer specialized care for residents with memory impairment.

Who We Serve: RCFEs serve older adults who need assistance with daily activities but do not require 24-hour skilled nursing care. Residents may include those with early to mid-stage dementia, mobility limitations, or chronic health conditions.

Licensing: RCFEs in California are typically licensed by the Community Care Licensing Division of the California Department of Social Services under Title 22 of the California Code of Regulations.

Payment Options: In California, Medicaid may help pay for assisted living through the Assisted Living Waiver (ALW). Eligibility is based on both financial and functional criteria. Contact California Department of Health Care Services or visit https://www.dhcs.ca.gov/services/ltc/Pages/AssistedLivingWaiver.aspx to learn about eligibility requirements and how to apply. Wait lists are common for waiver programs, so applying early is recommended.

Families researching assisted living options should verify the facility's current license status, review any inspection reports, and schedule a visit to assess whether the environment and services meet their loved one's needs. You can verify this facility's license and view inspection reports at California's facility search. For more information about assisted living regulations in California, visit California Department of Social Services, Community Care Licensing Division.

Licensing requirements and regulations may change. We recommend verifying current requirements directly with your state's licensing agency.

This place is money hungry. they will nickel and dime your loved one until they die or leave. I cant believe some of the things they charge for. If you are in need of assistance in this assisted care facility you pay extra. Example one my mom is in a wheel chair. if she did not feel well enough to go to the dining hall she would get charged extra for her meal to be broight to her. to go to the dinning room she has to wheel herself because if they assisted her down to the dinning hall she had to pay extra. The day she moved out some of the staff asked me not to put trash in the trash can upstairs as it gets too heavy for them. so they volunteered a large trash can on wheels. I thought great until they came and harassed her for $400 for using the dumpster they wanted us to use. If the person who was supposed to take care of her that day called is sick she just didn't get the care and left to fend for herself. do you think they offered a refund for services charged and not recieved, not a chance she had to fight for it. Money hungry and poor managment. Activities were the only good thing there.

When I began visiting assisted living facilities for my mom several months ago, Carlton Senior Living stood out as the best place for her to live not only safely but also to maintain much of her independence. Even though it took time for her to warm up to the idea of moving from her home in the Bay Area, where she had lived for 20 years, to an assisted living facility in Sacramento, she moved into Carlton 6 weeks ago and is now more content with her life than she's been in a very long time. She repeatedly tells me the staff is absolutely wonderful and helps her with whatever she needs with the utmost care, compassion, dignity and respect. Whenever she talks to any of the staff, she knows she is all that matters to them at that time and feels heard and valued. She also raves about the food. Honestly, I've never seen her eat as well as she has since moving into Carlton. As a result, she feels so much better both physically and emotionally. She has more energy throughout the day and loves participating in many different activities including those she never had an interest in before. My own experience with Carlton staff has been nothing short of amazing. Moving my mom into Carlton is the first time I've dealt with a senior living facility. I believe this experience has been so successful because everyone at Carlton has been there each step of the way with endless patience and open hearts. I'm deeply grateful to everyone at Carlton, and I think the text my mom sent me a couple of days ago says it best: "I love my life here beyond the moon!"

My 95-year-old mother was a resident at the Atrium at Carlton Senior Living for three weeks. Unfortunately, my mother passed away after a brief hospitalization, followed by her return to the Atrium on hospice care. I would like to complement all of the staff – maintenance, kitchen, nursing, care partners, med techs, housekeeping– for making my mother‘s brief stay as comfortable as possible. I would also like to thank them for their professional, kind and compassionate attitude towards my mother. The staff made a difficult situation a little easier for my husband and myself. We would highly recommend Carlton sr Living to family and friends looking for care for their loved one.

The problems we had with our family member residing at the Carlton Plaza in Sacramento began in July of 2013. The corporation did not realize that we had a family member working as a nurse, for an outside agency, within their facility. What my brother- in-law witnessed alarmed our entire family enough that we removed my grandmother from the Carlton. My brother-in-law observed patients in need of assistance who were not helped in a timely manner. He caught one of the night time medication technicians falsifying a medication record on his patient. He observed the supervisor of the memory care unit, playing on her office computer for extended periods, while there were residents with unattended needs and even one left in water soaked clothing for a lengthy duration. A family attorney made a concerted effort to reach out to the administrators of the Carlton Plaza in Sacramento in August to help resolve the misconduct going on in the facility but received no response. By September our family elevated our concerns to the owners of the Carlton Senior Living Corporation. We hoped that the two owners would take the issues seriously because one of them is supposedly a physician. The owners did not respond to our repeated requests to discuss the serious problems which were witnessed. The owners put all this stuff out online about how concerned they are with high quality care but they did not seem to care when we brought these serious issues to their attention. Why would they not care about one of their employees falsifying a record of administering medication? While moving our grandmother into her new home, my brother and I spoke with a retired Sacramento judge who had also experienced a nightmare at the Carlton before he also relocated himself to the Chateau at River's Edge. The judge told us of how he was doing rehabilitation at the skilled nursing section of the Carlton Plaza. Apparently the staff was so incompetent that they failed to follow orders which resulted in him falling through his shower bench and suffering irreparable knee damage. We would strongly encourage anyone who is considering placing a family member into an upscale assisted living facility to consider other options. The Carlton looks good on paper and has some great sounding reviews online but in reality the corporation is greedy and not responsive to serious concerns involving the quality of care and basic safety issues of residents. If your loved one does not really need much care, then perhaps the Carlton would be an okay but overpriced choice as an “independent living facility.” Our family would recommend avoiding the Carlton at all cost for any assisted living or skilled nursing needs.

DO NOT ALLOW YOUR SENIOR RELATIVE OR FRIEND TO GO TO THIS FACILITY! The management at the Carlton Plaza on Fulton is terrible. My family has filed approximately 15 complaints with the state licensing department in a four month period regarding my grandmother's care, safety, and living conditions. The facility was cited for falsifying my grandmother's paperwork in an effort to circumvent communication with the listed responsible party, my mother. The management then used that falsified paperwork as a means to communicate only with my grandmother who has dementia and was recently diagnosed with early Alzheimer's. The management is extremely unethical and manipulative...it is all about business and not about people and their families. A high level member of the management was even was recently reprimanded by her corporate office and mandated to take some type of sensitivity classes/seminars after talking about some residents to other residents in a nasty, negative manner (think middle school name calling!). Absolutely unacceptable! Failure to respond in a timely manner to my grandmother's emergency call button this spring also resulted in a citation by the state licensing department. Then three months later in June the Carlton completely failed to respond to my grandmother's emergency call button causing her to suffer in her apartment all night in horrific pain. Because this was a repeat offence in such a short period of time the Carlton's citation will be more serious. I could go on and on discussing the problems with this facility, but I think three major incidents verified and cited by the state licensing department is enough said...all between March and June. There are many other incidences that have occurred with other residents, my grandmother was not the only one. Whenever you visit elderly living facilities ask to see their DSS binder with the reports of all state complaints. Ask at the Carlton and you will see pages and pages of recent complaints. BEWARE OF THIS FACILITY! GO SOMEWHERE ELSE!!!
