Seasons Alzheimer’s Care and Assisted Living
Seasons Alzheimer’s Care and Assisted Living is a licensed assisted living facility located in San Antonio, Texas, specializing in memory care services for individuals with Alzheimer's disease and other forms of dementia. Assisted Living Facilities provide housing, meals, personal care, and support services in a residential setting designed to promote independence while providing needed assistance.
Services typically include assistance with activities of daily living (bathing, dressing, toileting, transferring, eating), medication management, meals, housekeeping, laundry, social and recreational activities, transportation, and 24-hour staff availability. Some facilities offer specialized memory care.
Who We Serve: Assisted Living Facilities serve older adults and people with disabilities who need help with daily activities but do not require the 24-hour skilled nursing care provided in nursing homes.
Memory Care Services: This facility offers specialized memory care services, including staff trained in dementia care techniques, activity programming designed for cognitive engagement, and appropriate safety measures. When evaluating memory care options, families should consider staff training, staff-to-resident ratios, security features, and the approach to managing behavioral symptoms.

Payment Options: In Texas, Medicaid may help pay for assisted living through the STAR+PLUS. Eligibility is based on both financial and functional criteria. Contact Texas Health and Human Services or visit https://www.hhs.texas.gov/services/health/medicaid-chip/programs-services/long-term-care to learn about eligibility requirements and how to apply. This is likely one of the hardest decisions you have to make when you need to move a loved one into a memory care. After research and visiting some memory care homes we decided Season's on Nacogodoches would be good as they had a brand new facility being built. We moved mom in shortly after it opened and for the first couple of months things "seemed" good but then it went downhill. There are many daily and nightly things that changed as the atmosphere is more of one that if the residents are in their rooms that's fine leave them there. Do I think the atmosphere is one of care and understanding that these residents are not themselves - No I don't. I had text messages that my mom was doing things they didn't like and somehow I was supposed to stop it. Wait a minute, she's there because she needs help and does things she doesn't know she's doing. Staff changes continually so you can't really get to know who is caring for your loved one along with most of them speak mainly Spanish which for my mom is definitely more than confusing and for us we needed either google translation or hand signals. I wouldn't mind the Spanish if they treated mom lovingly and not like a nuisance. I will say Marlene and Sandra were two bright spots. We did keep mom here for almost 2 years and I really regret not moving her sooner. Others we knew did move their loved ones much sooner. Things along the way that came up I would text or ask about and things would change for like a week and then go back. I asked twice about getting the families who come to visit together so we could have a support system for each other but that was a No and that's a red flag for me because why wouldn't you want that so you can support each other and have them check on your loved one if your out of town. I understand more clearly why that was a No. As I stated in the beginning, this was and is such a hard decision to make and if I could do it over this is not where I would have put my mom. I can't change the decision now but happy to say she's at a new place that we are very happy with and see more love and care for mom.
December 8,2024 Mona, I would like to personally thank you and all your staff for the excellent care and love my mom received while at Seasons. Mari, Arlene, Allison, Kendra, Margarita, Sylvia, Yamileth, Orlando and all the staff THANK YOU! From the first moment mom arrived to drawing her last breath, the staff loved her and assisted her to live her life to the fullest and leave this world gracefully and respectfully. Thank you.
I cannot express enough gratitude for the exceptional care at Seasons Alzheimer's Care. It is clear that this facility prioritizes the well-being and dignity of its residents. The staff is not only highly trained but also genuinely compassionate, making a challenging time much more manageable for our family. A special shout-out to Lenora! Her warmth and dedication made a world of difference. Lenora went above and beyond to ensure that my loved one felt comfortable and engaged. Her patience and understanding were evident every day, and it was heartwarming to see her interact with the residents, always with a smile and a kind word. Lenora truly embodies the spirit of care and compassion that Seasons stands for. I highly recommend Seasons Alzheimer's Care for anyone seeking a nurturing and supportive environment for their loved ones. Thank you, Seasons, and especially Lenora, for the incredible care you provide!
My sister was there for 19 months. The staff always make sure that she was always clean. The food looked and smelled great. The nurses are top notch. For my sister it was home when she couldn't be with me anymore. I recommend for people to stop in and check it out yourself.
We are very grateful for the care we received for our grandmother at the Season’s. We pick this facility because of the small community they offer so our grandmother received the highest care. She was available to do activities there and she love the food they served. The place clean and they kept our grandmother room very clean at all times. We can’t thank you them enough it’s hard to put a family member under someone else care but the service they provided was exceptional.
