Centerwell Home Health
Centerwell Home Health is a Medicare-certified home health agency serving Fort Smith, Arkansas and surrounding areas. A Medicare-certified home health agency provides skilled nursing services, physical therapy, occupational therapy, speech-language pathology, medical social services, and home health aide services to patients in their homes.
Medicare covers home health services for patients who are homebound and require skilled nursing care or therapy services on an intermittent basis. A physician must certify that the patient needs home health care and establish a plan of care.
Home health agencies are certified by the Centers for Medicare & Medicaid Services (CMS) and must meet federal Conditions of Participation. These requirements ensure that agencies provide quality care, maintain proper documentation, and protect patient rights.
Medicare beneficiaries pay no cost for covered home health services when provided by a Medicare-certified agency. For more information about home health benefits, visit Medicare.gov. Compare home health agency ratings at Medicare Care Compare.
Quality ratings and agency data are updated periodically by CMS. We recommend verifying current information at Medicare Care Compare.

The nurses and staff that worked with us were just wonderful. They were all very knowledgeable on what they were doing.
The nurses and physical therapists were professional and kind. However, had to spend some time in the hospital and thought we'd pick my care back up after I got back home. Got a call on a saturday that my nurse was coming out that day for a check up. No one showed up, then didn't hear anything after about continuing care until about 3 weeks later when I got a call for a quick checkup and finding out I was discharged from the program weeks prior. Never recieved a phone call or any type of notice that I was discharged. Came out 3 times, was a complete waste of time and waste of money. Quite frustrating, but is what it is. Will just have to get with the VA to set something else up.
The therapy group was absolutely wonderful. They have done in a little time what rehab at the nursing home couldn't in a lot of time. They haven't got my mom back to walking yet but do have her sitting up unassisted, doing sitting down exercises and if it wasn't for insurance I am positive for once in a long while they would have her up and walking again. They was more than nice and taught me a lot about how to help her even with the medical problems I have. They have helped not just her but me feel more comfortable about how to take care of her both physically and medically and dealt patiently with my high anxiety. The only thing I wish is insurance would give them just a little more time for physical therapy since she is finally making such great progress. I would definitely suggest them to anyone.
Everything has been wonderful! I feel very comfortable with the care I am receiving. The nurses are very kind and knowledgeable. Jennifer and Colt are both very good, but Jessica is excellent. She seems to truly care about your health and progress. I trust her completely.
They are great with my grandma and we don't want to lose them
