On Site Home Care

On-site Home Care is a licensed non-medical in-home care agency assisting families with companionship, personal care, hospital-to-home transition, transportation, respite care, and light housekeeping. Our services are generally provided to elder adults, individuals coping with Alzheimer's & Dementia, terminally ill, handicapped, or transitioning from one stage to another. Offering our clients and their families a sense of comfort and independence in a familiar environment through the delivery of skilled personal care, social engagement, transportation, and companionship is our goal while providing these services in the most economic, and efficient manner possible. Our team will work directly with you or your loved one, family members, and/ or professional care providers to develop a plan that is most suitable for the care recipient. ​​ All care beneficiaries will receive a customized plan of care explaining specific care details, equipment to be used, and responsibilities of the provider and the agency. This plan is created by the agency’s supervising Nurse during the client evaluation meeting and will be evaluated every quarter to ensure the implemented services are effective and in alinement with the clients personalized care goals.
