Synergy HomeCare and Lucent Hospice recently had the privilege of caring for a mutual patient. Coordination between our two teams was seamless — from hospital discharge and transportation to communication with the patient’s family and trusted neighbors. This collaboration exemplified whole-team attention at the bedside, ensuring that no aspect of care or comfort was ever overlooked. Upon arriving home, the patient was warmly received and immediately comforted by Synergy’s administrator, case manager, and caregiver, together with Lucent Hospice’s administrator, admission RN, RN case manager, and two devoted neighbors. It was a dignified homecoming — a true model of compassionate, coordinated care that should serve as the standard. We deeply value this partnership and look forward to working with Nia and her exceptional team again soon.

SYNERGY HomeCare is providing services to the my dad via a VA collaboration. My dad is 90 years old and has his ways, but he is very satisfied with the service that Synergy is providing. His really likes his personal care giver Eileen Kramer. She is very easy to work with and has a soothing personality which works fantastic for my dad. We are all very happy.

Rosemarie is by far the most trusted caregiver I’ve ever had walk into my home. She is very attentive, loving and caring towards my husband Luther. Not only does she care for him but also helps me around the home and that is a blessing. I wish there was more caregivers like her with the passion and thrive that she has and gives towards her patients and the family members surrounding him. She truly is one of a kind and I don’t know what Luther and I would do without her!
