Community Home Nursing Services is a Georgia licensed Private Home Care agency that has been caring for elderly and medically fragile clients in South Georgia since 1992. Eligible consumers may receive Georgia Medicaid-funded CCSP or SOURCE. Community Home Nursing Services is a provider of these two services: Personal Support Services - In-home services such as personal care, meal preparation, light housekeeping, shopping and in-home respite services. Structured Family Caregiver program pays the family member to care for their loved one in their home. Structured Family Caregiver is based upon eligibility and provides support to family caregivers who live with and assist an elderly and/or disabled waiver participant requiring significant assistance with activities of daily living.
