Swan Hospice is a Medicare-certified hospice provider serving Lakewood, New Jersey and surrounding areas. A Medicare-certified hospice provides comfort care and support services to patients with terminal illnesses and their families. Hospice care focuses on quality of life rather than curative treatment.
Hospice care is available to patients whose physician certifies they have a terminal illness with a life expectancy of six months or less if the illness runs its normal course. Patients may receive hospice care at home, in a hospice facility, nursing home, or hospital.
Medicare-certified hospices must provide all four levels of hospice care: routine home care, continuous home care for periods of crisis, general inpatient care for symptom management, and inpatient respite care to give caregivers a break. Care is provided by an interdisciplinary team including physicians, nurses, social workers, chaplains, and trained volunteers.

I am completely shocked at the treatment my mother received from this company. We trusted them to help us care for my mom, who is dying from stage 4 metastatic lung cancer that has spread to her brain. I trusted her care team and shared personal information about myself and my mother with them. In the midst of her decline, an aide thought she might need to go to the ER. This was later communicated to me but not to other members of her care team. The timeline of what happened after the ER suggestion does not make much sense. Also, no one told me that when a hospice patient is sent to the ER, their hospice care is rescinded. When she arrived at the ER, both the doctor and the nurse were confused as to why she was there. They explained that if the ultimate goal is hospice care — which it was and still is — then she should not be admitted, especially given her condition. However, they had no reason to admit her. Long story short, her hospice care was rescinded because they sent her to the ER, and then they refused to readmit her. I am beyond upset and deeply let down. Now, we have to find a new placement for her and a new care team. I would never have allowed her to be sent to the ER if I had known it meant hospice care would be terminated. Part of the reason this happened is due to a terrible lack of communication among the patient care team. There is little to no clear communication between the nurse, social worker, and aides. Make sure you document EVERYTHING and ensure that everyone on the patient’s team is fully informed about what is happening. It’s a shame that it becomes your responsibility to make sure the team communicates — especially when you are dealing with a loved one who is dying. I strongly urge others to document everything. This company is disorganized, and members of the same care team often find things out after the fact — or are unaware that something even happened. This company needs to take accountability for the lack of communication with my family and others, and they must make changes. I hope this post helps future patients and families, and I sincerely hope no one else is ever put in the position we are currently in.
The nurses and aides are good. The Dr. Is a little slow with filling prescriptions so recommend adding your regular Dr. As the one Dr you go to for everything else. The head management at swan sucks they don't know the difference between a young cancer patient whose dying and the elderly. They will release you the minute they don't see any decline in health period. The aides, nurses and social worker are the only good people there. They discuss the patients care or lack of non care in the future without discussing it with the patient or their representative present. I find that very unprofessional as I used to work in the healthcare field and social work field. That is very unperson centered management of the care of the patient.
I want to thank Sandra, an Aide from Swan Hospice, for the care and attention given to my loved one. People like Sandra are rare and hard to come by and I will be forever grateful to Sandra caring for him as if he were her own family member!
