Continuum Care Northbay, LLC is a Medicare-certified hospice provider serving Petaluma, California and surrounding areas. A Medicare-certified hospice provides comfort care and support services to patients with terminal illnesses and their families. Hospice care focuses on quality of life rather than curative treatment.
Hospice care is available to patients whose physician certifies they have a terminal illness with a life expectancy of six months or less if the illness runs its normal course. Patients may receive hospice care at home, in a hospice facility, nursing home, or hospital.
Medicare-certified hospices must provide all four levels of hospice care: routine home care, continuous home care for periods of crisis, general inpatient care for symptom management, and inpatient respite care to give caregivers a break. Care is provided by an interdisciplinary team including physicians, nurses, social workers, chaplains, and trained volunteers.

The team at Continuum provides compassionate, dignified, quality care to the patients and families on their services. Nurse Tarah, Nurse Noah, Chaplain Matt, LCSW Kay, and HHA Erica have been a delight to work side by side with while cohesively providing support and care to my residents.
I greatly appreciated the social worker and the staff who played music assigned to my mom's case. However, I am deeply unsatisfied with the nursing staff. Concerns and questions would often go ignored. Communication was poor. I never knew when they would see my mom and according to the owner of the place my mom is at told me that the nurse wouldn't even do vitals when she came to see her. I had to reach out for updatea. Not even physically evaluated by a doctor. She was denied recertification (thank goodness) because when I had an appointment with her regular doctor, he validated all of my concerns about her not being on a medication she had been on her whole life (hospice discontinued it) and that could be a contributing to her decline.
We have had a wonderful experience at our residential care facility with the entire Continuum team. They have been warm and caring to each of the residents who have been under their care. They are responsive when we have questions and readily available to give us help or advice. They all go above and beyond and we feel grateful for their attentiveness and compassionate care. Although it is not our decision which agency our residents go through when seeking hospice care, we will continue to recommend Continuum!
Yall actually have the rudest nurses I have ever met. I work in one of the memory care facilities in the area and regularly interact with your nurses and have had god awful experiences with most of them. Do better and retrain them on human decency. If this is how they treat staff at the facilities how are they treating their patients? Mind boggling.
This is a wonderful agency. They are genuine and empathetic to the dying and their families. I was so impressed with their gentle nature and the respect they showed their patients as well as family members. I am the daughter of one of their patients. I whole heartedly recommend this agency!
