Perpetual Care Hospice is a Medicare-certified hospice provider serving Simi Valley, California and surrounding areas. A Medicare-certified hospice provides comfort care and support services to patients with terminal illnesses and their families. Hospice care focuses on quality of life rather than curative treatment.
Hospice care is available to patients whose physician certifies they have a terminal illness with a life expectancy of six months or less if the illness runs its normal course. Patients may receive hospice care at home, in a hospice facility, nursing home, or hospital.
Medicare-certified hospices must provide all four levels of hospice care: routine home care, continuous home care for periods of crisis, general inpatient care for symptom management, and inpatient respite care to give caregivers a break. Care is provided by an interdisciplinary team including physicians, nurses, social workers, chaplains, and trained volunteers.

When our mom entered hospice, we had several choices for care. Kaiser Hospital told us they have seen good results with several, and we are so glad that we selected Perpetual Healthcare. Our mom lived with Alzheimer's for eight years, giving us time to know her wishes pretty well. After falling at home and going through evaluation at the hospital, her broken back gave her intense pain, so we took her home for hospice care. Perpetual sent personnel immediately to help us get acquainted with Mom's care. In fact, the nurse arrived before the ambulance brought our mom home! Elaine, our RN, gave the family a four hour class, teaching us how to use the equipment provided, and how to understand our mom's medication needs. Blanca, our home-health aide, came twice per week to bathe our mom so she could be more comfortable. The entire staff that we interacted with from Perpetual was there for us at every turn. Medications were delivered within a few hours, and if we had questions, our phone calls were always answered by their staff...not an answering service! Overall, Perpetual helped us help our mom be comfortable for the last nine days of her life, and with the procedure after her death. We will always be grateful for comfort and ease as our mom slipped into the hands of God.
My mom was with Kaiser. I asked her Dr. For a referral on a Monday by Tuesday I got a call from Perpetual Hospice care. That same day a nurse, equipment and a hospice social worker came. It was so amazing the care my mom received. They were so comforting to me as well. Every time I needed something for my mom it came right away. They were literally there 24hrs a day. They also checked in on me to make sure I was ok as well. They went above and beyond everything I could have expected. When my mom passed away the nurse came in a timely manner and all the equipment was taken care of as well the same day. Perpetual Hospice care was fabulous. Best experience for such a difficult time in my life.
Perpetual Care Hospice provided the care for my sister for the last six weeks of her life. she was 89, suffering from a recurring bout of breast cancer and advancing dementia. I was attempting to provide for her as best I could long distance as I live in Texas (Dallas) and she is in Lancaster, CA. She was very weak and frail and had had a couple of bad falls in her apartment prior to being hospitalized. Concluding she could no longer return to her apartment the staff at Kaiser Panorama City Hospital recommended Hospice care at an attending care facility. From my first contact with Churchill at Perpetual to the follow up stay at JR Retirement Home, they were all very caring and empathetic. Arlene kept me posted with any issues and concerns as they came up....I did not have to call her. They secured the bed that was needed at her Boarding Care facility as well as weekly visiting nurses and on call physicians. All of this filled me with confidence that my sister was in the best hands possible. I cannot thank them enough for their caring and gentle care that they provided along with the great staff and facilities at JR Retirement Home. She passed from this earth on February 24, 2024.
My Dad died recently and we had the good fortune of being connected with Perpetual Care Hospice through my Dad's medical care team. I can't speak for the rest of the staff as I did not meet them but I can certainly speak to our experience with the continuous care nurses and their support team. Jo and Tessa were my Dad's nurses. Both of those women came and brought such a level of care and compassion that was woven with strong, gentle, loving guidance as well as knowing when to step back to let my family take the lead. Their attention to every detail of my Dad's care was amazing to watch. In the quiet moments of conversation, they were very attentive to conversation at hand but they always kept an eye on my Dad. The way they explained everything that was happening brought so much peace to a very stressful situation. It made making the hard decisions of my Dad's care easier when we understood what was happening and what our choices were. Thank you Ms. Jo and Ms. Tessa for the care that you brought and for your guidance and compassion during my Dad's transition. You made a difficult time in my family's life easier to handle and for that we will always be grateful.
