Registered Nurse (RN) Job Description
Job Summary Serra Center is seeking RN candidates for our 3 licensed residential care group homes located in Fremont. The homes provide 24-hour care, supervision, and support to individuals with intellectual disabilities who have a recurring but intermittent need for nursing services. This is a great opportunity for Nurses seeking part-time or consultant work in a residential care environment. We offer a casual and professional environment, with flexible work schedules ranging from 6-10 hours per week. With over 47 years of experience and community relationships, Serra Center’s dedicated team of nurses, physicians and program professionals work directly with each individual and their family to develop a personal care plan that focuses on each individual’s medical and support care needs, while facilitating individual choice, goals and dreams. Responsibilities and Duties The Registered Nurse is responsible for integrating, coordinating, and monitoring the medical care and treatment process. The RN is instrumental in the interdisciplinary team and fulfills the following roles: *RN Authority: Pre-admit and admission evaluation, periodic re-evaluation of services, discharge plan. Make judgments regarding client health issues within the scope of the Nurse Practice Act Section 76878(b). *RN Oversight: Visits the facility for health services and client health assessment as needed but no less than one hour per client per week. *Teaching: Client Personal Hygiene, family life, sex ed. Direct Care Staff in-service training related to physician ordered care and client Nursing care plans, care protocols. *Nursing Services Plan & Evaluation: Adhere to facility written plan for provision of all nursing services; provide evaluation and recommendations for modification as required. Review client Care Plans quarterly. *Drug Regimen Review: Drug regimen is on a monthly cycle, which is reviewed and checked-in monthly and as changes occur. *Medication Administration: Oversees the administration of medication by Direct Care Staff, who are authorized to pass medication under the RN’s license after successful completion of a medication course and exam. Qualifications and Skills The ideal candidate will possess exceptional skills with attention to detail, performing assessments, care triage, and making independent decisions related to client care needs. *Self-initiative and ability to work well with others in a team-oriented environment. *Accept and respond to phone calls as required, related to client changes-in-condition. *Ability to prioritize responsibilities and manage time efficiently and effectively. *Thorough and detailed oral and written communications skills. *Reliable and Insured transportation. *Valid California RN License & ability to pass DOJ Clearance.
