This position: -assumes responsibility and accountability for the practice of professional nursing in a manner consistent with the NH State Nurse Practice Act and American Nurses Association Standards for Home Health Nursing. -implements teaching appropriate to the client’s needs and within the context of his or her environment and value system. -practices nursing with respect for individual, cultural and spiritual differences. -coordinates delivery of care along health care continuum utilizing community resources. -demonstrates teamwork and effective communication to accomplish client, team and agency goals. Qualifications: -Graduate of a State licensed basic nursing program -Preparation in Community Staff Nursing through a National League for Nursing approved program; -One (1) year professional nursing experience under appropriate nursing supervision; or -A combination of education and experience that demonstrates the person has acquired the necessary competencies required for the position. -Possession of a current State of New Hampshire license to practice as a licensed -practical nurse. Maintain current cardiopulmonary resuscitation (CPR) certification. -Must be able and willing to learn to utilize a portable electronic field device. Must be able to manage electronic patient health information (ePHI) remote access in accordance with HCS' policies and procedures. -Sensitivity in human relationships. Able to cope with family emotional stress and tolerant of individual lifestyles. Skill in using psychological principles in both one on one and group situations. Emotional stability, flexibility and respect for various religious and spiritual beliefs. -Valid driver’s license and access to an automobile during working hours that meet the State of New Hampshire registration regulations and auto liability insurance.
