RN Case Manager

Locally owned and operated Home Health Agency is hiring a part-time and full-time RN. Responsible for providing 1-1 patient care in their homes based on Dr's orders. Create and individualized plan of care in coordination with physician and other agency disciplines. Must be willing to travel to patient homes in Boone and surrounding counties. Very flexible schedule, benefits, work independently. Agency has been established since 2006.
