We are seeking a passionate RN Case Manager for Wake County and surrounding areas. This position is Full-Time Monday- Friday. (Negotiable) RN Case Managers visits 5-6 patients on any given week (schedules negotiable on Friday before 11:00 AM), driving to the patient's home and providing/supervising skilled or non-skilled nursing services. This position is primarily responsible for the initial assessment and case development of patients in all stages of life in their homes. Duties include but not limited to, assessing, interpreting, planning, implementing, and evaluating patients according to the patient’s age and diagnosis. Registered Nurses (RNs) with strong assessment, skilled nursing, and documentation skills are strongly encouraged to apply! If this sounds like you, we'd love to hear from you! Must be able to check your email intermittently during the week to respond to documentation requests. We are 100% EMR and clinicians document in the patients' homes with company-issued laptops.
