Skilled Nursing needed in Riverdale

We are seeking LPNs to join our Home care team! Responsibilities: Care of a 59 year old male in his home Administer nursing care to a 59 year old male in his home. Establish patient treatment plans as ordered by a physician. Monitor and report changes in patient symptoms or behavior Communicate with collaborating physicians or specialists regarding patient care Educate patients about health maintenance and disease prevention Facilitate referrals to other healthcare professionals and medical facilities Maintain accurate patient medical records Provide advice and emotional support to patients and their family members Qualifications: Previous experience in nursing Familiarity with medical equipment like vent, feeding tube, wound vac, trach care. Ability to build rapport with patients Strong problem solving and critical thinking skills
