RN/ Case Manager

Integrated Senior Care Home Health and Hospice is looking for a motivated, independent, and organized RN to be a case managing Nurse for both Home Health and Hospice patients. This position would be coordinating care with several other modalities to provide the optimal care plan for the individuals health challenges. Excellent communication skills will be key for this position. As the Case Manager, you will be the center point in coordinating care and communication with the patient, the family, the Primary Care Provider, the facility(if applicable) the Therapy Team, and all other moving components that can be used to great the best continuum of care for each individual. You will perform under the direction of the DCS(Director of Clinical Services) and have access to any additional training and education that may be needed.
