Certified Nursing Assistant (CNA)

MINIMUM QUALIFICATIONS: High school diploma or equivalent; successful completion of a course in home health or adequate on-the-job training in another approved facility utilizing basic skills; graduation from an accredited Nurse’s Aide Course with current Certified Nurse’s Aide certificate; current CPR certification; and valid driver’s license, personal vehicle for on-the-job transportation, and evidence of auto insurance coverage. Essential duties and responsibilities Certified Nursing Aide includes the following but is not limited to: Provides emotional and physical support to terminally ill patient, family, caregiver, and patient authorized representative. Assists in improving the terminally ill individual’s quality of life during the final stage of life. Provides assistance to others who are caring for terminally ill patients in the home. Maintains and promotes the independence of the patient and family in maintaining safe and healthy environment. Assist with personal care activities including, but not limited to: Bathing (tub, showers, bed bath) Mouth care, denture care, skin care, foot care. Grooming (shampoo, dry and comb hair, shave, ordinary care of nails). Transferring (bed to chair, wheel chair, to tub, to shower) Dressing and undressing activities Ambulation Turning patient in bed and re-positioning for comfort. Assists with household tasks directly essential to the patient's health including: Measures and records oral, axillary temperatures, pulse, respiration and blood pressure. Assisting in feeding patient if appropriate. Light house keeping of the patients immediate environment changing linen, making bed, common laundering of patients clothing and bedding. Meets safety needs of patients including safe use of equipment. (footstools, side rails, step stools, wheelchairs, chairs, oxygen, etc.) Assist the patient in self administration of medications by reminding the patient to take their medications when requested. Never to directly administer medications. Performs or assist patients to perform activities in accordance with the hospice aide care plan as taught and supervised by the registered nurse and in adherence to State and Federal regulations. Immediately reports any changes patient, family, caregiver, or patients authorized representative or incidents to the Case Manager and/or Director of Patient Care Services. Accurately documents services provided in accordance with Solace Hospice’s Policies and Procedures on the day services are rendered. Participates in staff development programs as appropriate for hospice aides. Assumes personal responsibility to participate in discipline appropriate education to meet the annual education requirements 12 hours/12 months) Accepts responsibility for maintaining skills, learning and adhering to the hospice’s Policy and Procedures on an ongoing basis. Other duties as assigned by the supervisor. Must be able to function in the patient’s environment with minimal direct supervision, accepting personal responsibility for maintaining a professional relationship with the hospice’s patients, family, caregiver and patients authorized representative. Ability to meet hospice’s conditions of employment, health clearances and provisions of references.
