Certified Nursing Assistant (CNA) Job Description

SUPERVISOR: Charge Nurse/ Director of Nursing DEPARTMENT: Nursing Service JOB SUMMARY: Provide each of your assigned residents with routine daily nursing care, activities of daily living and other services in accordance with the resident’s plan of care. ESSENTIAL FUNCTIONS: 1. Obtain vital signs, measure oxygen saturation, weigh patient as needed, measure height of patient as needed, maintain therapeutic environment, and document patient care/action. 2. Promptly responds to patient, family, and staff needs and concerns as appropriate. Investigates suggestions and works with departmental leadership to find solutions. Effectively prioritizes works and deals with multiple demands and tasks. Can work independently within scope of practice under the direct supervision of the licensed nurse. Demonstrates the ability to be flexible and organized. Can work effectively as a team member. 3. Meets the patient’s personal care needs by providing basic nursing care: bathing, toileting, grooming. Assists with feeding and hydration with attention to maintaining and promoting adequate/prescribed intake and recording output. Provides basic skin care with attention to ambulating, positioning, turning, and transporting. Provides restorative care through activities of daily living, performing range of motion, and using assistive devices properly. Assists in a bowel and bladder program. Plays an active role in maintaining an environment of safety. 4. Maintains therapeutic environment for patient and ensures that information about the patient is consistently communicated to the Charge Nurse. Immediately reports abnormal results or findings to the Charge Nurse. Accurately and timely documents vital signs, activities and care delivered. 5. Greets and directs patients and visitors. Answers and directs telephone calls. Maintains confidentiality of the resident’s health information. Communicates admission, transfer and discharge information according to established procedures to appropriate disciplines. Facilitates communication with outside agencies as necessary. 6. At all times maintains professional image and behavior towards co-workers, hospital, medical staff, visitors and residents. Demonstrates awareness of his/her public images in interpersonal relationships in the workplace. Communication, cooperation and assistance are given promptly in a clear and courteous manner. Consistently adheres to the confidentiality policy. Communicates effectively with residents, their families, staff, colleagues and physicians. Supports the mission, philosophy, and goals of the nursing home. 7. Takes responsibility for duties performed on the job. Is accountable and reliable for assigned shifts and maintains a positive attitude. Treats all residents with kindness, dignity and respect. 8. Other duties as assigned. PHYSICAL/MENTAL DEMANDS/ ENVIRONMENT: Must be able to lift 25 lbs to a height of 5 ft and carry for a distance of 50 ft. This position frequently requires the ability to push, lift, lower residents; lift, lower, push, carry supplies; and lift, lower, push, and carry equipment. Must be able to stand, walk, sit, stoop, kneel, squat, climb, balance; reach above the shoulder, reach below the shoulder; grasp, finger, handle, feel objects; have visual acuity both near and far, depth perception, color discrimination, and peripheral vision. Must be able to communicate verbally and in writing and be able to hear residents, blood pressures, alarms and the nurse call system. Must have the ability to prioritize and perform multiple tasks. The work environment is primarily indoors with occasional outside activity. This position involves shift work to cover services 24 hours a day 7 days a week. Environmental conditions include occasional exposure to chemicals, gasses, fumes, dust, radiation, vibrations and noise. There is an inherent potential for contact with blood, body fluids or tissues. $13.50 - $18.00 per hour
