Licensed Practical Nurse (LPN) Job Description

Job Summary: The Licensed Practical Nurse or Licensed Vocational Nurse provides nursing care to patients in the home as prescribed by the physician and in compliance with applicable laws and regulations and the policies of the company. Responsibilities: - Drives to prospective patients’ residences. - Ensures quality and safe delivery of home health services, including education and information to patients, families, and caregivers pertinent to diagnosis and plan of care. - Implements the plan of care initiated by the case manager. - Provides accurate and timely documentation consistent with the plan of care. - Monitors patients’ condition including fluid intake and output and composes patient charts. - Assist ailing patients in daily necessary activities such as eating, bathing, etc. - Administer injections, prescribed medications, enemas, etc. - Clean and dress wounds and assist with other basic medical care tasks. - Collects samples for lab testing as assigned. - Participates in the coordination of home health services, appropriately reporting the need for disciplines (CNA, OT, PT, MSW, etc.) to the case manager and/or clinical supervisor. - Uses equipment and supplies effectively and efficiently. - Performs other duties as assigned by the RN. - Perform duties consistent with the Nursing Practice Act including the Standards of Competent Performance. Qualifications: - Hold a current Licensed Practical Nurse (LPN) in the State of Nevada. - Graduate of an accredited practical nurse or vocational nursing program. - At least 6 months of experience in Home health care preferred. - Experience in wound care, vacs care, ostomy care, and IV administration is preferred. - CPR certification. Job Requirements: - Must have a valid driver’s license and auto insurance. - Required to present a negative TB test and other tests as required by agency policy. - Must be able to do fieldwork. - Must have excellent written and verbal skills. - Excellent planning, organization, and presentation skills. - Ability to think and work independently and with direction and communicate with staff members in a fast-paced and sometimes stressful environment.
