Licensed Practical Nurse (LPN) Job Description

As a Skilled LPN with Care With Love, you will play a vital role in this team, offering thoughtful, one-on-one care in clients' homes, supporting each client’s unique goals, and ensuring they feel truly seen, heard, and cared for.

The Skilled LPN:
Travels to patients’ homes to provide direct patient care and implements the plan of care initiated by the Registered Nurse.
Provides accurate and timely documentation of visits consistent with the plan of care.
Assesses and provides patient and family/caregiver education and information pertinent to the diagnosis and plan of care.
Participates in the coordination of home health services, appropriately reporting the identified needs for other disciplines (HHA, OT, PT, MSW, ST) to the Clinical Manager.
Participates in the peer consultation process; communicates with the Clinical Manager and other clinicians on the team to provide updates, insight, and recommendations.
Actively participates in a high-performance environment to not only drive change but truly enhance the quality of patient service.
Participates in personal and professional growth and development.
Qualifications
Graduate of an accredited practical nurse or vocational nursing program and is currently licensed as a licensed practical nurse or licensed vocational nurse in Virginia.
Has 2 years of nursing experience. Community/home health or medical/surgical experience is preferred.
Must be a licensed driver with an automobile in good working order.
Possesses and maintains current CPR certification.
Demonstrates excellent observation and problem-solving skills.
Provides open communication with active listening.
Shows the ability to follow directions, stay organized, and prioritize workload.
