Care Finders Total Care, a Modivcare Personal Care Service, is looking for an RN or LPN responsible for providing high-quality services to our home care private duty clients. We are seeking a compassionate, reliable Licensed Practical Nurse (LPN) or Registered Nurse (RN) to provide one-on-one nursing care for a 15-year-old teen with a seizure disorder. This role is ideal for a nurse who enjoys building meaningful connections, maintaining a calm presence, and supporting a teen’s safety, independence, and quality of life in a private setting.

Pay: LPN hourly rate $35 , RN hourly rate $40
Schedule: 2 days per week Mon & Tues 7:30a-5p

Responsibilities
- Monitor for seizure activity and respond appropriately per established care plan
- Administer medications as prescribed and observe for side effects
- Maintain accurate documentation of care, seizures, and interventions
- Ensure patient safety during daily activities
- Provide calm reassurance and support during and after seizure episodes
- Communicate effectively with family, care team, and supervisors
- Support the teen’s routine, comfort, and dignity
- Follow physician orders and individualized care plan at all times
Qualifications
- Active LPN or RN license in good standing
- Experience with seizure disorders preferred (training considered for the right candidate)
- CPR/BLS certification
- Strong assessment and critical-thinking skills
- Dependable, punctual, and attentive
- Comfortable working independently in a private-duty setting
- Compassionate, patient, and respectful when working with adolescents
