Home Health LVN Visiting Nurse

We are a Medicare & Joint Commission accredited Home Health Agency currently hiring Licensed Vocational Nurse (LVN) to do an intermittent visits in Los Angeles Area , Burbank and San Fernando Valley for our patients. We use Data Soft Logic for our software (DSL) Core Function : Renders general nursing care to patients in Home Health setting. Job Details: LVN to do follow up visits according to the plan of care initiated by Registered Nurse (RN) LVN provide servicers requiring substantial specialized nursing skills LVN to initiate appropriate preventive and rehabilitative nursing procedure LVN inform RN, Physicians, Case Manager and other appropriate personnel of changes in patients conditions and needs LVN instruct the patient and family in meeting nursing related services LVN prepare clinical and progress note, and participate in-service training programs Note: Rate is per visits depending on experience
