Hospice/HH LVN/Field Nurse in Fontana, CA
Hospice Of Ie And OcLicensed Vocational Nurse (LVN) Job Description
We have a case in Fontana, CA 92336 visits once to twice a week. Visits patients homes' assisting with and providing patient care. The field lvn participates in patient and family teaching, communicates and works in conjunction with other disciplines in promoting and maintaining independence and optimal health to homebound patient. The field lvn is an integral part of the interdisciplinary team and will be giving report to the office nurse to provide the best plan of care for the patient. New graduates/New to home health welcome. Paid hourly rate for orientation.
