JEWISH ASSOCIATION ON AGING JOB DESCRIPTION Job Title: Registered Nurse-Case Manager Division: Sivitz Hospice Department: ***** Effective/Revision Date: 11/01/2009 JOB SUMMARY: The Registered Nurse Case Manager is a responsible as a member of the interdisciplinary group who works under the general direction of the Medical Director and/or the attending physician in the provision of skilled nursing assessment, planning and coordination of services in order to maximize the comfort and health of patients and families. JOB DUTIES / RESPONSIBILITIES: 1. Assumes responsibility for a patient/family caseload that includes the assessing, planning, implementation and evaluation phases of the nursing process. 2. Initiates communication with attending physicians, other hospice team members, and agencies as needed to coordinate optimal care and use of resources for the patient/family. . 3. Performs initial assessment and obtains data on physical, psychological, social and spiritual factors that may influence patient/family health status and communicate that data in team meetings to formulate an Interdisciplinary Plan of Care. 4. Maintains up to date patient records so that problems, plans, actions and goals are accurately and clearly stated and changes are reflected as they occur. 5. Coordinates physical care of the patient and instruct primary caregivers, volunteers, home health aides and Licensed Practical Nurses in providing direct care as appropriate. 6. Attends interdisciplinary group meetings to facilitate discussion of patient/family status in order to evaluate and revise the interdisciplinary group plan of care. 7. Supervises and documents the home health aide plan of care every two weeks. 8. Shares in providing 24 hour on-call support 7 days a week. 9. Provides appropriate support at time of death and period of bereavement. 10. Participates in orientation and inservice training programs for hospice staff. 11. Participates in Quality Improvement Program activities. 12. Assists with the education of contracting facilities and community members as well as meeting with physicians and groups to market hospice services. 13. Represents the Hospice in professional and community activities as requested. 14. Demonstrate familiarity with policies of the agency and rules and regulations of 15. State and Federal bodies which aid in determining policies. 16. Performs other related functions as requested. REQUIRED EDUCATION, TRAINING AND EXPERIENCE: 1. Graduate of a school of nursing accredited by the National League of Nursing, 2. Baccalaureate degree in nursing preferred. 3. Registered, certified or licensed as required by State Law on the profession. 4. Pennsylvania driver’s license, current auto insurance and access to a car during working hours. 5. Hospice /Home Health experience with terminally ill patients is preferable. 6. Certified Hospice and Palliative Care Nurse preferred. REQUIRED SKILLS AND ABILITIES: 1. Demonstrate flexibility, excellent assessment skills, ability to supervise others and the confidence to work independently. 2. Communicate effectively within a team. 3. Manage time efficiently. 4. Show concern for the provision of quality services. 5. Demonstrate skill in nursing practice, ability to cope with patient/family emotional stress and tolerance of individual lifestyles. 6. Ability to do public speaking, educational programs and community outreach. 7. Ability to maintain confidentiality of information 8. Basic Computer Skills PHYSICAL REQUIREMENTS: 1. Lifting: Regularly lifts and moves patients, equipment, patient supplies. Frequently lifts up to 40 pounds. Occasionally lifts and moves patients unassisted up to 50 pounds. 2. Carrying: Carries medical supplies, and other items, weighing up to 30 pounds. 3. Pushing/Pulling: Pushing and pulling is
