A very hands on gentle and caring community for the residents. I am proud to be associated with Jackson Creek Memory community.
Terrible communication from admitting. Never saw social worker as she scheduled the meeting and never attended. Grandma was discharged back to her home, but not even to get up by herself, toilet herself or dress herself. LPN "diagnosed" UTI, gave the lab paperwork to us, that was for another resident. BIG HIPPA violations. Refused to assess for stroke like symptoms stating "she already has a scan scheduled tomorrow". This is just a few the issues.
My room was sufficient with privacy curtain. Large bathroom easy to maneuver. Staff very helpful and willing to help you achieve your goals, but it was up to you to accomplish them. The day shift was especially helpful if need arose.The Physical and Occupational Therapist were professional and knowledgeable. They were also patient and encouraging. Experience was helpful and I would use again if necessary.
My mother was admitted to Villages Of Jackson Creek, 3980 S. Jackson Dr. Independence, Mo. 64057 for rehab on Friday, February 7, 2025. On Monday, February 10, 2025 my sister went to see our mom and ask our mother where was her phone. Mom said it was on the bed. After searching for several minutes and couldn’t find it, my sister went to the nurses station and ask about the phone. She was ask to describe the phone. The employee showed my sister a phone and it was my mother’s. The phone was cracked and wouldn’t turn on. The employee said that the phone was in the laundry and got washed. Apparently whoever took the linen off the bed didn’t pay attention to the phone on the bed. My sister spoke with several employees about this issue and was told that the facility is responsibility for the phone. The social worker did a report three days later and said she would have an answer today. She never contacted my sister but I went to the facility and ask her about it. She told me that the facility was not responsible for the phone. I’m very upset because whoever took the linen off the bed didn’t check to see if there was anything on the bed before removing the linen. The lack of accountability is upsetting because the phone was the only way we could community with mom and the social worked showed no concern or compassion. Also, we ask the facility to NOT DO her laundry because we will do it and a note was placed on mom’s door. Someone put my mom’s clothes in the laundry. I don’t know if all of the employee’s working there are competent enough to be working at that type of facility. I feel that the facility should replace her phone that they are responsible for washing.
My husband was sent here after bad care at the hospital after his total knee replacement. I was skeptical as I had heard some bad things about this facility in the past. But I was pleasantly surprised. He received wonderful care, everyone was so nice. Shout out to Terita and Victoria, they were great! The only reason I didn't give 5 stars was because of the food. Apparently the food is good if you're able to ambulate to the dining room, but he couldn't. He did not get any choices in the food brought to the room, and it left a lot to be desired. But that's a small issue, and overall we were very happy with the care he received!
