If zero stars were an option, that would of been my selection. Severly understaffed, wounds being ignored. People screaming all over and being ignored. While I visited I was asked by a nurse to watch a patient while she cared for someone else. On Day 2 of visiting I walked into a lounge area and found a woman hunched over begging me to.help her. She desperately wanted to be moved from a dining room chair back to her wheelchair because she was in pain. I immediately let a nurse know and the nurses exact words were " OH my gosh, I didn't even know she was back here. " I didn't even realize she was missing!!!! My heart was shattered and more fear seeped in. PLEASE DONT PUT YOUR LOVED ONES HERE! Yes I have proof.

Several things! The Staff , I can't say enough about how wonderful they are! My Mom is treated with kindness and respect. The facility itself is warm and inviting and feels like home. The residents, always smiling and very content. That says a a lot to me and this is where I can feel so comfortable leaving my loved one in their care and know she will be taken care of the best!

I was here for a short term stay for my physical therapy and my occupational therapy. The staff is absolutely amazing from the minute you are brought to the center. Very clean facility, organized and the rooms make you feel at home. They also have many activities to participate in. Once I'm discharged to go home I will miss the staff and friends I made here at Three Oaks. Nikki👍

Do not put your loved ones here! They will be constantly alone, unstimulated, left unclean with out daily cares done! I am a trained professional and have received many clients from them with bed sores that took over year to heal. If you do decide to put them there expect to be there everyday so they get the attention and care they need.
