One of the worst-rated facilities around—and for good reason. Like many others have noted, this place has a dismal track record, including a 1-star rating from Medicare. Incredibly, Medicare has cut off funding to this facility three times. That alone should be a major red flag for anyone considering this place for themselves or a loved one. The Facility: The physical environment is in poor condition—dirty carpets, stained floors, broken furniture, and malfunctioning wheelchairs. The building also has a persistent odor that makes being inside unpleasant. While masks aren’t required, I’d still recommend wearing one just to help block the smell. The Care: Call lights are often ignored for hours. I saw patients left in hallways for long stretches, seemingly forgotten. Basic needs like hydration, food, and medication administration appear to be inconsistently met. I overheard patients complaining that their medications were delayed—or didn’t seem correct. It was deeply concerning. The Staff: Unfortunately, the staff reflect the facility’s overall low standards. Many I encountered were unprofessional, unhelpful, or outright rude. But the real problem is leadership. What little “management” I observed seemed disinterested and disengaged. Rather than attending to patient needs or staff oversight, they were frequently spotted on breaks, smoking or socializing. Accountability: Given the alarming number of enforcement actions and fines listed in state records—including allegations of neglect and medication errors—I would strongly urge anyone considering this place to do thorough research. If you must interact with this facility, document everything and consider consulting a legal or medical advocate. Summary: This is not a safe or dignified place for vulnerable individuals. I saw nothing here that would give me confidence in the quality of care provided. If you have other options, I strongly recommend you explore them.

Please do yourself and your loved ones a favor and avoid this place at all costs. Do not come here! Very low quality facility and staff. But don't take my word for it. Here is what the regulatory agencies think of this place: The Centers for Medicare & Medicaid Services (CMS) and state authorities conduct regular health, fire, and safety inspections, typically every 15 months, with the most recent inspection on April 4, 2024. Key findings include: Health Inspection Rating: 1 star (out of 5), placing Sunrise Health Services in the bottom 20% of facilities nationally, compared to Wisconsin’s average of 2.78 and the national average of 2.77. In other words, don't come here unless you have absolutely no other choice. Addendum: The response from the owner is disingenuous. They’ve been posting basically the same thing for years if you look at prior reviews and yet things never change. Clearly, they don’t realize that the “Executive Director” is part of the problem. Sunrise is an organization that is rotten from the head down. The entire management team there is composed of liars. They’ll lie to your face, they’ll lie about you, they’ll lie to others, they’ll lie behind your back. Very quickly you’ll learn they are not worth talking to. As for the “nursing management team”. Good luck with that. Look for them smoking out on the patio. That’s where they spend most of their time. Too bad they don’t spend a little less time smoking and little more time on patient care and patient management. Maybe, just maybe, Sunrise wouldn’t be the total disaster that it is if it had capable, qualified, competent, and caring management. I’ll say it again. Stay away from this place. Check all the publicly available information online from the governmental agencies, and you’ll learn all you need to know. Unfortunately, it's too late for my family member, but I hope anyone who reads this does what's best for their family members and avoids this place like the plague!

Most of The CNA’s at the facility we’re lazy and rude. There’s a handful of good ones but for the most part, this is a horrible place to be. I was left alone in the dining room because I am wheelchair bound until they were closing the dining room down the kitchen. Staff was leaving. Nobody came to get me. I asked about five people nobody came. They don’t answer the buzzer that’s in your room that you press just a horrible experience. The food was disgusting. The social worker was rude. I was just a horrible experience. Don’t go there. Don’t send your family member there.

This place is on the National Nursing Home Watchlist for many reasons, and I wish I had known that ahead of time. I strongly suggest you check the NHAA website for detail. Mostly it's just an awful place: awful facility, awful staff from top to bottom, awful care, although I hesitate to use the word "care" since there's very little of that going on. I wouldn't trust these "health care professionals", who are anything but professional and seem unconcerned with providing healthcare, to clean the cat box, let alone handle any of my or my family's health care needs. My real rating is Minus 10 stars, but the lowest I can give is one.

SUNRISE Health Services was a God Send to our family! It is a hidden gem right in our area that we never knew existed. Our father was brought to a facility in Burlington, WI from up north. I will not mention the name, but it was terrible. We went to look at 3 places in Milwaukee with Sunrise being the first to visit. Lauren Beaumont Executive Director is a fabulous person to speak to. Very friendly, caring and knowledgeable. Needless to say, we went to the other 2 facilities and then headed right back to Sunrise. They had an open bed, and we had our father at Sunrise the same day. The staff are very good and caring. They would keep our father's tray in his room and warm it up later for him if he didn't eat and feed him if necessary. They took extra care in checking up on him and making sure he was positioned in bed properly. We met with the PT/OT team, and they were very motivated to help him and would take him every morning until he couldn't anymore. Sad to say our father was 93 and passed away towards the end of October 2024. We are very GRATEFUL for the 2 months we were able to spend visiting with our father. And especially Grateful for finding Sunrise Health Services! Thank you for all you do!!
