You need to know before you decide to take your loved ones here. I have noticed most of the REVIEWS here are either, reviews because they work here or the residential area. My Father got admitted on Friday and during weekends NO WATER in the facility like no water at all, if the resident go number 2? Cant flush which we find it disgusting, i have never heard a facility without water, not only that NO AC, that is why they provided ELECTRIC FAN IN EVERY ROOM A VERY DIRTY FAN.. can you imagine..also floors are not maintainely clean, Please do not get confused, i was because I didn’t know the floors yet. This facility is mixed with assisted living other floors and skilled nursing facility which is ONLY IN 6th FLOOR. Other floors are assisted living with different name. This is our experienced base on our Father’s stays from 5-23-2025 to 5-30-2025 at PIONEERS HOUSE 6th floor. The CNA’s some of them just doesn’t care beware of the cna name JOSH, we were told he is lazy , not caring at all, left our father soiled and left the soiled pads on the floor smelly and unsanitary. Other CNA WE ARE MOST ENCOUNTERED WITH LIKE Ms. Arlene, Ms. Elizabeth, Ms. Elena they are compassionate, caring and shows that they care same as For NURSES : Mr. MARK specially you, your the best. AND Ms. Catalina we are so grateful for you all, we can’t thank you enough for taking care of our Father even in such a short period of time. We also thank the wound care nurse, your awesome, THE PT DEPARTMENT thank you so much for being kind and caring to our Father. So before you decide if this is the right facility for your family try to drive around and visit. Also before and after the discharge planning there is a lot of confusion. Make sure to communicate and visit your love ones every day.

Pioneer House is well ran, especially the activities coordinator Norma. She really tries to make it well for the residents. They try to do occasional events and make it special on the holidays. The building is for seniors, and on the top floor, there are more nurses and staff to care for those who have greater physical need. I don't have any family here, but I am part of a group that does community service a few times a month, and the residents are truly appreciative. Feel free to call and ask a lot of questions. Finding a health center, senior home, nursing home for your loved one is not something that should be rushed, but I do believe Pioneer is a solid place for residents- especially those who have a more critical need. There is a small parking lot in the back, and there is usually some street parking on the weekends. Wishing you the best.

This Skilled nursing facility has the most exceptional staff. From the moment you enter the 6th floor, every single person is friendly, goes above and beyond to make the residents feel comfortable and well taken care of. The therapy department is very knowledgeable, compassionate and attentive. They take the time to understand the residents needs, and create a special plan in order to help with the recovery. Also, the facility is clean and very welcoming. High recommend if you are looking for the best care for your family member!

I'm BLOWN AWAY !!! I love downtown Sacramento. Sac-Town. And Pioneer House is the place for Rehabilitation. My family was in the BEST care there... I loved being able to be so close to everything and knowing so many resources were available within walking distance when he transitioned into his senior housing nearby. Thank you to the amazing staff. Great therapy team too. Even the food was good. Here's to Pioneer House !!!

My mother has been living in this facility for several years. I believe the great care she receives keeps her going strong. Many thanks to the entire staff for giving such great care. I would especially like to thank Sandy Mala, the Social Services Manager for keeping me informed with Mother's needs and changes in care or routine.
