Definitely a great skilled nursing facility. As soon as you walk through the front door you are met with a clean welcoming environment. The CNA’s are very knowledgeable and very respectful to the patients they are caring for. Social services is also amazing, they make sure every patients needs/rights are being carried out during their stay and even getting them care at home if needed. If you or a loved one is in need of care rest assured this is the best option and solution for your worries. They have an outstanding team that has the proper resources to facilitate any and all matters. Our family is so grateful for the wonderful care and support my dad received during his two-month stay at Pacific Heights Transitional Care Center. From the nurses, the physical, speech, and occupational therapists, to the activity coordinators, case manager, and social worker - every single staff member went above and beyond to make sure he felt cared for, encouraged, and respected. After my dads stroke, and losing mobility on one side, we weren’t sure what his recovery would look like. Thanks to the dedicated therapy he received three times a day, five days a week, we started to see good progress that gave us hope and strength. The team not only supported him, but genuinely welcomed our entire family. They even adored my daughters, who grew fond of everyone during our daily visits—we all miss them dearly! Most importantly, it gave us peace of mind knowing my dad was in such good hands even when we couldn’t be there. The kindness, patience, and professionalism of the staff will never go unnoticed. Pacific Heights will always hold a special place in our hearts.
My mom stayed at Pacific Heights Transitional Care Center for about three months after a severe hemorrhagic stroke on October 13, 2024. Before her stroke, she was an incredibly active 57-year-old who had just hiked Half Dome. Her decline was sudden and devastating. She needed an emergency craniectomy, was intubated, and spent weeks unable to open her eyes, speak, sit up, or even hold her head up. She then lived three and a half months without part of her skull before undergoing a cranioplasty. It was an unimaginably intense medical journey. When she first came home, she had complete left-side hemiplegia, needed to be pushed everywhere in a wheelchair, and our two flights of stairs made daily care extremely unsafe. Nearly six months post-stroke, a home health PT told us he did not believe she would walk again. But around that time, she began showing the smallest motor return in her left hip, and we knew we had to act quickly, even though we didn’t have the capacity or expertise at home to build on it. After months of caring for her ourselves, including literally carrying her down our stairs in her wheelchair to get her to outpatient therapy, we became completely burnt out. Admitting her to Pacific Heights Transitional Care Center was one of the hardest but best decisions we made. The therapy team helped her build on the early movement she had regained and relearn the foundations of walking, including weight shifting, stepping, balance, and confidence. Their consistency and structure turned small gains into real functional progress. After three months, she went from total dependence to now walking at home with a walker and the support of one person. She can navigate stairs with assistance, something that once felt impossible, and she is well on her way to doing these things independently in the coming months. The staff also trained us as caregivers, teaching my husband and me how to support her walking and stair practice safely. That education made a tremendous difference. Throughout her stay, the nurses, CNAs, and therapists created a warm, attentive, and respectful environment where she always felt safe and cared for. A special thank you to Jasmine from physical therapy, Joanna from speech therapy, Miranda from occupational therapy, and nurses like Massami, along with Diana and Kenny, who all showed her kindness and genuine care during her stay. Pacific Heights Transitional Care Center played a crucial role in giving my mom strength, mobility, and hope again. I wholeheartedly recommend this facility to any family navigating the overwhelming world of post-stroke recovery. They made a profound difference in my mom’s life and in ours.
My father is currently in this facility. The nurses and staff here are kind, helpful and genuinely want the best for your loved ones. I appreciate the timely updates and notification of any changes to my father's condition, medication, Or general care. I am always updated and feel at ease knowing my father is in good hands here!
I visited this facility at least 5 times in the last 3 months and my interaction with the staff has been wonderful. As soon as you walked in the receptionist will great you and have you sign in. She is very friendly to all that comes in. The restroom in this facility is always clean. I spoke with Social Worker Shirley last time I visited and she is referring a home health client for the company I work for. She is always dressed nicely , speaking softly at all times and friendly to everyone. My observation while I was there, nurses and caregivers are attentive to patient. I would recommend this facility to you and your love ones.
