Continuity Care Home Nurses

My grandfather received excellent care from the nurses at Continuity Care Home Nurses. He was recovering from TKR, and a nurse came daily to change the dressings. The scheduling was perfectly constructed around our day-to-day needs and the staff had great bedside manner.

The caregivers here are caring, knowledgeable and professional. I continue to use Continuity Care Home Nurses on an as needed basis at this time. I recommend them to anyone that needs care giving services.

Continuity Home Care has been a godsend in the care of my great aunt. She has been on service with them twice and we have had several caregivers. Each has been polite, compassionate and extremely ethical. My aunt has done well because of their dedicated care. I have peace of mind knowing that this reputable agency is looking after my aunt. Continuity's nursing staff (LVN's and RN's) has cared for a member of my family for about 4 years now. He suffered a catastrophic brain injury in 2014 and requires 24-hr nursing care. My family member is considered a "complex care" patient because of the fact that he has a tracheostomy, he is tube fed, and he has other more complicated issues. We have developed a deep respect for Continuity Home Care over the years because of the many good experiences we have had with them caring for our family member. While no nursing agency is perfect, Continuity's approach--under the steady leadership of its wonderful owner--is to make the client happy by providing heartfelt home nursing care for individuals that need it. On the rare occasion where a particular nurse may have fallen short of the company's standards, Continuity has always taken accountability and gone out of their way to find us other nurses to pick up the slack. Our relationship with the office staff has been strong (Doris comes to mind). We've also maintained a strong relationship with the owner throughout these last several years. What sets this company apart from some of its competitors, is its tireless approach to finding and providing the highest quality nurses it can for its clients, and ensuring that those nurses maintain a high quality of care on an ongoing basis. If you have a loved one that needs a higher level of care in the home, I highly recommend Continuity Home Care because they truly do care about their clients, and that includes both patients and their families.

CCHN was there when we needed them the most! Our family member was injured on the job site and we needed post operative care and CCHN came to the rescue.
