Home Care Partners, a Division of Professional Nurse Partners was founded by Debbie as part of her determination to help to make a difference in the lives of those individuals needing quality care. Debbie chose to open a licensed home care agency that complemented the Life Care Management Organization that was created to support aging adults and their families navigate the challenges they faced with changes in their health condition(s). Debbie also was faced with making decisions for her own loved ones years ago and realized her passion and clinical experience caring or her own family could be shared with others. Professional Nurse Partners and Home Care Partners were established in 2011. We have a team of registered nurses, client care coordinators, health promotion specialists and compassionate, experienced home care support staff. We apply a holistic, integrative approach to providing exceptional care to those clients we serve. It is our privilege to be part of those individuals and their families lives.
