Gracious Care Hospice is a Medicare-certified hospice provider serving Beaverton, Oregon and surrounding areas. A Medicare-certified hospice provides comfort care and support services to patients with terminal illnesses and their families. Hospice care focuses on quality of life rather than curative treatment.
Hospice care is available to patients whose physician certifies they have a terminal illness with a life expectancy of six months or less if the illness runs its normal course. Patients may receive hospice care at home, in a hospice facility, nursing home, or hospital.
Medicare-certified hospices must provide all four levels of hospice care: routine home care, continuous home care for periods of crisis, general inpatient care for symptom management, and inpatient respite care to give caregivers a break. Care is provided by an interdisciplinary team including physicians, nurses, social workers, chaplains, and trained volunteers.

Gracious Care Hospice assisted with end of life care for my Step Mother and my father over the last 2+ years. They were incredible to work with. The staff educates and advocates for the patients in their care and educates the family to help with decision making. They have numerous resources to provide extra interaction for the patients in their care. I would refer them knowing without a doubt that the patients and family are in the best of hands during the most difficult time.
Absolutely incredible team. Placed the call on Monday about my parent, had the assessment done on Wednesday, and by Wednesday afternoon, she was admitted to the hospice program. Incredibly compassionate and organized team, and a special shout out to Kathryn Blanchon (and Alyssa) who made all of this happen thoughtfully, quickly, and with amazing communication skills. Our family was blessed with locating Gracious Care during my parents transition to hospice care. Let me save you time before looking anywhere else, start here.The quality of care, attention to detail of medical and nutritional needs is second to none. The home is very clean, updated to a level which anyone would be happy to call home. Before my father passed, we were able to take walks at the nature trail less than 5 minutes away, go out for meals with family and friends, with an occasional stop at Dairy Queen for his favorite blizzard. My mother is still in their care and she praises the kindness of the staff. She also loves the visits from her care team i.e. nurses, chaplain, musician, bathing, painting finger and toenails just to name a few. Thank you Gracious Care for all you do.
Both of my parents entered hospice care with Gracious Care Hospice on the same day. This transition was expected for my Mom, but unexpected for my Dad. It was an overwhelming time of change. I'm especially directing this review towards the hospice team that came into my patents's home to provide hands on care. We were very fortunate to have Alyssa, Trey, and La Sarita help us out. They were excellent communicators and very caring and competent. They exhibited great teamwork. They addressed our questions without making us feel like we were wasting their time. An additional note. After my Dad passed, just 13 days after entering hospice, my Mom did not want to sleep in her bedroom alone. Gracious Care Hospice made arrangements so we could keep my Dad's rented hospital bed so I could sleep next to her and hold her hand as she grieved the loss of her husband of 68 years. That act was outside of the hospice routine, but was a key to helping my Mom pass peacefully, 17 days later. Thank you, Gracious Care Hospice.
