Aseracare Hospice is a Medicare-certified hospice provider serving Valparaiso, Indiana and surrounding areas. A Medicare-certified hospice provides comfort care and support services to patients with terminal illnesses and their families. Hospice care focuses on quality of life rather than curative treatment.
Hospice care is available to patients whose physician certifies they have a terminal illness with a life expectancy of six months or less if the illness runs its normal course. Patients may receive hospice care at home, in a hospice facility, nursing home, or hospital.
Medicare-certified hospices must provide all four levels of hospice care: routine home care, continuous home care for periods of crisis, general inpatient care for symptom management, and inpatient respite care to give caregivers a break. Care is provided by an interdisciplinary team including physicians, nurses, social workers, chaplains, and trained volunteers.

They took great care of my dad 2 years ago. They we're all so kind and offered so much. Lynn was his nurse and always had a joke for him. That's why I called them when my mother needed to come home on hospice care. Alyssa was my mother's nurse. So kind and caring. It takes a special person to care for the dying on a daily basis.
My mother and I recently decided that it was time for hospice for her. Our initial visit was such a life giving experience, as Lynn shared compassionately and kindly what my mother would have available to her as she journeys toward bringing her life to a closure. The admitting nurse Debbie was so informative and caring, and took time to listen, learn, and inform us. Since then, my mother gets the best care from kind and caring professionals. Not only are her emotional, spiritual, and physical needs met, this team ministers to my needs as well. I am so glad we chose AseraCare! My mother loves her nurses Debbie and Heather, and her Aid Jen is fabulous! Lindsey is a great social worker and we look forward to meeting Chaplain Kelley soon. Thanks to AseraCare, we are experiencing this journey with such beauty and grace.
I would like to extend a profound thank you to Aseracare, hospice nurse Kaitlan and Director of Operations , Lynn Criggley for the exceptional care extended to my mother to ease her condition during the transition process. It is great to have these caregivers who possess this extraordinary compassion and our family is grateful for their presence at such a difficult time. While the time spent caring for my mom was rather brief, we appreciate their care and skill during this critical time.
What a godsend AseraCare was for my father. The nurses were so kind and helpful, both to my dad and to us (his daughter, my mom, and my brother). They went above and beyond- going from tender patient care to being a support team and giving us pep talks to get through our difficult time and dealing with difficult family members. They made certain his comfort was priority, and made sure we had enough medications and care items to get through. I will forever be thankful to the lovely nurses and staff for everything they did
My family had a great experience with AsceraCare hospice. Once the decision was made to put my mom in hospice, they had the bed at her house within hours. I don't know how they got everything done so quickly,, but they made it happen.. The nurse and case worker that handled everything with our family were wonderful and caring. They went over everything with us in detail. The nurse even called multiple pharmacies for us to locate which one had the meds we needed that night for her. (My only recommendation is to provide a sheet of paper for the family/caretakers to list and keep track of dosage amounts and schedule of medications.) Sadly my mom passed less than 12 hours in hospice. The nurse was at our house within an hour of her death. I had to call hospice a couple times that night with questions and my phone call was always returned right away. We got supplies shortly after she was admitted to hospice (but because of her quick passing, we were not in need of them anymore). Our caseworker even showed up at my mom's wake with a gift for the family. The chaplain has called each immediate family member at least once within the first month of her passing, and we have his number if we are in need of any support.
