Forest Plaza POSITION DESCRIPTION Position Title: Direct Care Worker / Medication Manager Reports to: Service Coordinator / Registered Nurse Position Purpose, Authority, and Accountability: Under the general direction of the Service Coordinator (SC), the Medication Manager / D.C.W. provides personal care, medication administration of non-parenteral medication and supervision to tenants in a manner conducive to their safety, comfort, security and the greatest degree of independence possible. The Medication Manager/Companion operates within the parameters of each tenant’s service plan and daily schedule as determined by the tenant, facility situation, the SC discretion, and within the practice limitations of training. Minimal Education/Training/Licensure/Certification Requirements: High School Diploma or GED; 18 years or older. Minimal Skill or Proficiency Requirements: Functional literacy in spoken and written English. Demonstrated effective interpersonal and communication skills. Working knowledge of Assisted Living regulations, and/or willingness & ability to learn. Demonstrated skills in working with elderly. Essential Job Functions: A. TENANT CARE FUNCTIONS 1. Assists in the implementation of each tenant’s service plan by performing personal cares (for example: dressing, bathing, grooming, toileting) to assigned tenants according to their service plan and daily schedules of care, and in such a way as to promote safety, comfort, security and the greatest degree of independence possible for each individual. 2. Documents according to established policies and procedures the care and treatment provided and tenant response or lack of response to that care. 3. Observes and verbally reports to the nurse on duty accurate and timely information concerning assigned tenants, such as change in medical condition or health status, lack or response or resistance to care, special problems, unusual incidents, tenant needs or requests, behavioral or psychosocial concerns; documents in writing according to established procedures or as requested. 4. Safely administers or supervises self-administration of medications in accordance to Forest Plaza policies and procedures; asking appropriate questions and/or getting assistance as needed. Staff monitor for drug reactions and report tenant complaints of health concerns to the nurse. Reports non-routine findings to person-in-charge. 5. Complies with procedures for safe lifting, transferring, and handling of tenants; assumes responsibility for safety of tenants under immediate supervision or in caseload assignment at any given time; reports and documents incidents/injuries according to established procedures. B. DIETARY FUNCTIONS 1. Performs various food preparation/covering/labeling tasks as assigned. 2. Prepares and serves beverages per recipes and/or instructions. 3. Neatly arranges table settings with proper tableware, condiments, etc. 4. Serves meals to tenants, in accordance with established schedules, dietary procedures, and tenants’ rights/quality of life principles. 5. Delivers nourishment's/supplements to specified tenants as scheduled. 6. Monitors dining room during meals; observes and records consumption (when/if directed by the nurse on duty or Lifestyle Coordinator) and requests for/receipt of substitutions; observes and responds to needs for substitutions or other assistance with dining; 7. Clears tables and removes dishes/trays from dining room to dishwashing room. Sanitizes dining room tables, sweeps/mops floors as needed. 8. Assists dietary staff as needed/directed. C. ENVIRONMENTAL FUNCTIONS 1. Performs general cleaning tasks such as sweeping, dusting, washing windows, vacuuming, cleaning light fixtures, cleaning bathrooms. 2. Performs floor care such as dust-mop, wet-mop, and spot-mop. 3. Empties trash containers, replaces liners as appropriate and disposes of trash in proper receptacle.
