Caregiver Job Description
• Performs the task assigned by the RN Supervisor
• Must be present at MANDATORY RN SUPERVISED VISITS every 50- 60 days. You are to be available for a 2- hour window when these visits are scheduled.
• Assisting the client with getting in and out of bed
• Care of eyeglasses and hearing aids.
• Assist the client with walking and ambulation of walker, cane, or crutches.
• Changing the clients’ bed and laundering the bed linens and the clients’ personal clothing.
• Assist the client with skin care excluding wound care.
• Assist the client with toileting including the use and care of bedpan, urinal, commode, or toilet.
• Report in writing to the RN Supervisor on each assignment
• Promptly report any significant changes observed or reported in the client’s condition to the RN Supervisor.
• Assist the client with simple transfers, including bed to chair or wheelchair and reverse.
• Reminding client’s to take Medication. You may not administer medication unless authorized to do so by the RN Supervisor.
• Assisting with hair care, oral hygiene, bathing, grooming and dressing/undressing, helping with tying shoes, and with all personal care.
• Preparing, food purchasing, and planning meals to specific dietary requirements of the client and feeding if necessary.
• Assisting client with therapy exercises under the direction of a Registered Therapist ONLY.
• Assisting with toileting, routine bodily functions.
• Light housekeeping including dusting, vacuuming, changing bed linens, cleaning the bathroom, kitchen and laundry, only in the areas where personal care services take place, and also maintain a safe and healthy environment.
• Preparing daily records and submitting to the office by due date.
• Participating in education programs/training.
• Grocery shopping or errands as needed.
• Performing all tasks as indicated in the care plan.
• Providing companionship, and accompanying the client to doctor’s appointments to obtain medical diagnosis and treatment.
• Assist in making doctors or other appointments as needed.
• Report within 24 hours if a client is admitted to the hospital or if the client has any serious illness or injury.
• Report to the RN Supervisor or Administrator if the client or if you have any complaints or Grievances.
• Your job description may change at any time. The administrator or RN Supervisor may change your job description at any time and when advised there has been a change either in writing or verbally you are expected to make that change immediately.
