PENICK VILLAGE Job Description Home Care In-Home Aide Department: Home Care Job Title: Home Care In-Home Aide Reports To: Home Care RN Supervisors A. Job Duties and Responsibilities: 1. Provides hands-on care and performs tasks following the client Plan of Care. 2. Responsible for client care monitoring and communication with appropriate team members and supervisor(s). Responsible for following agency policy regarding procedures or duties that an aide may perform. 3. Follows the plan of care specifying the level of assistance required by the aide for each ADL task. 4. Help with prescribed exercises the client and aides have been taught by a health care practitioner 5. Assist client with self-administration of medications that are ordered by a health care practitioner or other person authorized by state law to prescribe. 6. Perform/assist with ADLs. 7. Record and report changes in the client's condition. 8. Promotes a desirable work environment/attitude among fellow staff members. • Conveys respect for staff and clientele. • Maintains a positive attitude at all times. 9. Displays a degree of flexibility in work assignments. • Acts as a team member and assists other team members to meet work requirements. • Maintains a flexible schedule to meet the organizations work requirements. • Attends mandatory in-service training programs. 10. Accepts client assignments and displays flexibility with schedule. 11. Follow the client’s plan of care. • Performs and or provides care/tasks specific to client needs following the client Plan of Care. Demonstrates safe aide skills and competence. • Report to the RN Supervisor if you are unable to perform a skill or competence. • Notifies RN Supervisor immediately as needed with any client problems or change in status. 12. Documents according to established agency procedure. • Documents daily the tasks performed, care provided, and any communication concerning client. • Utilizes time in the Home Care Scheduling Portal. 13. Maintains a clean and safe working environment. • Follows all standards for universal precautions, infection control and agency policies when reporting to assignments. 14. Maintains an accurate accounting of supplies. • Follows agency procedure regarding accountability for client and agency supplies. • B. Qualifications: 1. High school graduate or GED. 2. Must demonstrate proof of competency to the Home Care RN by return demonstration of assigned tasks. 3. Ability to work independently to creatively solve a broad range of problems, make sound judgments and work collaboratively with co-workers and clients. 4. Ability to be flexible and innovative. 5. Good interpersonal skills. 6. Speak and write the English language understandably. 7. Valid NC driver license, a safe personal vehicle in good working order and adequate automobile insurance. 8. Have smart phone to access and complete required documentation. C. Physical Requirements: 1. Ability to stand for extended periods. 2. Lift up to 50 lbs. 3. Ability to reach above, below or at shoulder height, and bend, stoop, push and pull. D. Attendance and Punctuality: 1. Gives proper notice when late or absent. 2. Ability to work weekend shifts with a minimum standard of one weekend a month and potentially every other weekend. 3. A work week that includes a Holiday each year. 4. Occasional overtime for schedule coverage and special events. E. Safety: 1. Understands and observes general health and safety guidelines. 2. Use of Physical Protection Equipment (PPE) as necessary. 3. Reports unsafe working conditions or unsafe conditions in a client’s home.
