About Us: Heart and Soul Healthcare, LLC is a new and emerging GAPP provider agency, dedicated to ensuring that individuals entering members' homes accomplish tasks in a timely and competent manner while maintaining the rights, safety, choice, and dignity of each member in accordance with the physician's plan of treatment and the agency's plan of care. As a new agency, we are passionate about building a strong foundation of quality care and service for our medically fragile patients. We are currently seeking a dedicated and experienced Clinical Manager (RN Supervisor) to join our team. The ideal candidate will have extensive knowledge of home management, skilled nursing/PSS care, and the ability to supervise and coordinate the work of others, make decisions, and communicate effectively. The GAPP provider agency is seeking a qualified Clinical Manager to join our team. The Clinical Manager will be responsible for ensuring that individuals entering members’ homes accomplish tasks timely and competently while maintaining the rights, safety, choice, and dignity of each member in accordance with the physician’s plan of treatment and the agency’s plan of care that is established for each member. Qualifications: A current Georgia RN license in accordance with the Georgia Registered Professional Nurse Practice Act, O.C.G.A. 43-26-1 et seq. Experience with medically fragile patients and home health is preferred. Knowledge of current methods of home management and skilled nursing/PSS care. Working knowledge of Rules and Regulations for Private Home Care Providers Chapter 290-5-54. Working knowledge of GAPP policies and procedures. Responsibilities: Monitor and supervise the care of members receiving GAPP services. Review referrals for the GAPP, evaluate members' skilled needs, and assign appropriate staff. Develop and revise member care plans, reviewing content during supervisory visits. Supervise personal care support personnel, with LPN co-signing and dating supervisory visits. Complete supervisory assessments and ensure proper documentation. Arrange for orientation of personal care support personnel or LPN to GAPP members. Complete monthly supervisory assessments. Adhere to all regulations and policies related to GAPP and private home care providers.
