Licensed Practical Nurse (LPN) Job Description
Job Summary We are seeking for Licensed Practical Nurse (LPN) candidates who are compassionate, caring, reliable, self-motivated, detail-oriented, flexible to work in different settings, excellent communication skills with experience in Home Health. The Licensed Practical Nurse will participate and assist the RN in implementing the developed plan of care of the patients. Responsibilities • Perform tasks under the direction of the RN, assist in identifying the client's physical, psychosocial, and environmental needs as evidenced by documentation, clinical visit notes, case conference with other clinicians. • Perform follow-up skilled nursing visits scheduled by the RN. • Administer medication as prescribed by the patient’s physician. • Discuss on-going care with the RN. • Collaborate with interdisciplinary teams (such as Physical Therapist, Occupational Therapist, Home Health Aide and others, including physicians and case manager) to ensure coordinated care. • Educate patients and their families about treatment plans and home care strategies. • Monitor patient progress on a continuous reporting basis to the RN. • Document and complete patient visit notes and necessary documentation in a timely manner to assure compliance with company policy and Home Health standards. • Participate in discharge planning with RN to ensure a smooth transition for patients’ post-treatment. • Demonstrate commitment and professional growth by participating in in-service programs and maintaining/improving competency. • Ensure that all safety and infection control practices are followed. • Respond to emergency situations. • Follow policies and procedures and Maintain with State and Federal regulations. Skills & Qualifications • Valid Florida State Licensed Practical Nurse. • Current CPR certification. • Experience in treating patients in home settings preferred. • OASIS knowledge a plus. • Flexible to learn and adapt to new software and applications. • Possess strong Problem solving and critical thinking skills. . • Excellent communication skills for educating patients and collaborating with other healthcare teams. • Ability to work independently with flexible schedule and to travel locally. All positions require a Level 2 background screening.
