Registered Nurse (RN) Job Description
Job Summary We are seeking for Registered Nurse candidates who are compassionate, caring, reliable, self-motivated, detail-oriented, flexible to work in different settings, excellent communication skills with experience in Home Health. The Registered Nurse will be responsible for evaluating patient needs, creating clinical assessments to admit, re-certify, discharge and transfer patients, coordinating with other clinicians and developing treatment plans. Responsibilities • Perform Initial Assessment and Evaluation visit based on Physician's order. • Develop individualized treatment plans based on patient evaluations and goals. • Create comprehensive assessments to determine the Plan of Care by identifying the physical, emotional, psychosocial, and environmental needs of the patients, including vital signs and medication review. • Schedule frequency of follow-up skilled nursing visits. • Confer with Physician in developing the initial plan of treatment. • Administer medication as prescribed by the patient’s physician. • Discuss on-going care and coordinate any changes with Clinical Manager. • Collaborate with interdisciplinary teams (such as Physical Therapist, Occupational Therapist, Home Health Aide and others, including physicians and case manager) to ensure coordinated care. • Educate patients and their families about treatment plans, home care strategies and procedures, provide recommendations for devices, equipment or tools that might improve the patient’s quality of life at home. • Monitor patient progress and adjust treatment plans as necessary with approval of the Physician and on a continuous reporting basis to the Clinical Manager. • Document and complete patient evaluations, plan of care and visit notes, any necessary documentation, and discharge summaries in a timely manner to assure compliance with company policy and Home Health standards. • Participate in discharge planning to ensure a smooth transition for patients’ post-treatment. • Demonstrate commitment and professional growth by participating in in-service programs and maintaining/improving competency. • Ensure that all safety and infection control practices are followed. • Respond to emergency situations. • Supervise as needed LPNs, CNAs and HHA's. • Follow policies and procedures and Maintain with State and Federal regulations. Skills & Qualifications • Valid Florida State RN License. • Current CPR certification • Experience in treating patients in home settings preferred. • OASIS knowledge a plus. • Flexible to learn and adapt to new software and applications. • Possess strong Problem solving and critical thinking skills. . • Excellent communication skills for educating patients and collaborating with other healthcare teams. • Ability to work independently with flexible schedule and to travel locally. All positions require a Level 2 background screening.
