Are you a Registered Nurse who is looking for a rewarding position that enables you to assist individuals to transition from long-term care placement back into the community and to provide on-going case management and nursing services for these individuals? HHCM is looking for Full-time and/or Part-time RN Case Managers to provide care for our patients in our transitional/long term care programs in the Sacramento County area. Other areas to include Davis and Dixon. Home and Health Care Management is an innovative company that focuses on case management services for individuals who, without assistance, would be living in long-term care facilities. You will be that person who coordinates services to allow our patients to remain in community living. This position allows you to get to know your patients and spend time working with them, one-on-one, on an on-going basis. You will provide skilled nursing assessments, medication management services, coordination of care with patients’ physicians and medical care team and supervision of our home health aides. There is limited hands-on nursing with this position. The hours are Monday through Friday during day-time hours. (But hours can be flexible after the orientation period). There is no on-call required. We will train for the position.
